清下解胰方对重症急性胰腺炎大鼠血清细胞因子的调节作用

【摘要】 目的:探讨清下解胰方对重症急性胰腺炎大鼠血清促炎细胞因子(TNF-αI、L-6)和抗炎细胞因子(IL-10)的调节作用。方法:将清洁级雄性SD大鼠随机分为对照组、模型A组、治疗A组、模型B组、治疗B组;通过经典胆胰管逆行注射法并改良建立重症急性胰腺炎大鼠模型,肉眼大体观察各组大鼠术后表现及死亡情况,光镜下观察胰腺病理损害的改变,检测血清淀粉酶及TNF-αI、L-6I、L-10水平,并分析各指标的相关性。结果:镜下观察胰腺的病理损害显示,治疗A组、治疗B组分别较模型A组、模型B组显著减轻(P<0.01)。模型A组、治疗A组与对照组相比,淀粉酶及TNF-αI、L-6水平均有显著性差异(P<0.01),且治疗A组较模型A组显著降低(P<0.01);模型B组较模型A组淀粉酶水平显著降低,而TNF-αI、L-6水平显著升高(P<0.01);治疗B组较模型B组淀粉酶及TNF-αI、L-6水平均降低,其中TNF-αI、L-6有显著差异(P<0.01)。模型A组、治疗A组与对照组相比I,L-10有显著性差异(P<0.01),且治疗A组和模型B组较模型A组显著升高(P<0.01),治疗B组较模型B组显著升高(P<0.01)。经相关性分析,淀粉酶水平与病理损害无相关;各组TNF-αI、L-6的变化情况一致,呈正相关,且与病理损害呈正相关;IL-10与病理损害和TNF-α无相关。结论:清下解胰方可能通过降低TNF-α与IL-6水平、升高IL-10水平调节促炎细胞因子和抗炎细胞因子,维持高水平的动态网络平衡,阻断"瀑布样级联反应"而起到减轻胰腺损伤的作用。

【Abstract】 Objective: To explore the regulative influence of "Qingxia Jieyi Decoction" on serumal proinflammatory cytokines(TNF-α and IL-6) and antiinflammatory cytokines(IL-10) of rats with severe acute pancreatitis(SAP).Methods: The clean male SD rats were randomly divided into control group,model A group,treatment A group,model B group,and treatment B group.The SAP model of rat was induced by retrograde injection of sodium taurocholate into the biliopancreatic duct.The rat’s reflection of each group after the operation and their death information were observed by naked eyes,the pathologic lesions of pancreas were observed through the microscope,the levels of serumal amylase,TNF-α,IL-6 and IL-10 were detected,and the correlation of above indexes was analyzed.Results: As for the pancreatic pathologic lesion observed through microscope,the treatment A group and treatment B group alleviated significantly compared with model A group and model B group respectively(P<0.01).The levels of amylase,TNF-α and IL-6 of model A group and treatment A group increased obviously compared with control group(P<0.01),and the levels of above indexes of treatment A group were lower than those of model A group(P<0.01);the level of amylase of model B group was lower than that of model A group,while the levels of TNF-α and IL-6 of model B group increased remarkably compared with model A group(P<0.01);the levels of amylase,TNF-α and IL-6 of treatment B group decreased compared with model B group,and there were significant differences in the levels of TNF-α and IL-6 between two groups(P<0.01).The level of IL-10 of model A group and treatment A group increased obviously compared with control group(P<0.01),the level of IL-10 of treatment A group and model B group was higher than that of model A group(P<0.01),and the level of IL-10 of treatment B group was higher than that of model B group(P<0.01).The correlation analysis showed that the level of amylase had no correlation with the pathologic lesion;the levels of TNF-α and IL-6 of all the groups had the same change with positive correlation,and the levels of TNF-α and IL-6 had positive correlation with the pathologic lesion;the level of IL-10 had no correlation with pathologic lesion and TNF-α.Conclusion: "Qingxia Jieyi Decoction" can relieve the pathologic lesion of the pancreas in rats with SAP,and its mechanisms may have relationship with regulating proinflammatory cytokines and antiinflammatory cytokines,maintaining the high level of the dynamic net balance and blocking the "cascade reaction" through decreasing the levels of TNF-α and IL-6 and increasing the level of IL-10.
