非小细胞肺癌患者瘦素、OB-Rb、VEGF的表达及其临床意义

【摘要】 目的探讨非小细胞肺癌(NSCLC)患者瘦素、瘦素受体(OB-Rb)和血管内皮生长因子(VEGF)表达的临床意义。方法应用免疫组化法检测30例NSCLC患者的病变组织组织及癌旁正常肺组织中的瘦素、OB-Rb和VEGF,将其表达与NSCLC临床病理特征指标进行相关分析。采用多因素Cox回归分析瘦素、OB-Rb和VEGF表达水平与患者预后的关系。结果 NSCLC组织中瘦素、OB-Rb和VEGF的阳性表达率均高于癌旁正常组织(P均<0.05);瘦素、OB-Rb和VEGF表达与NSCLC的病理类型、淋巴结转移和TNM分期均无相关性(P均>0.05);OB-Rb、VEGF表达与NSCLC预后无相关性(P>0.05),TNM分期和瘦素是影响NSCLC患者预后的独立风险因素(P<0.05)。结论瘦素与OB-Rb结合后,可能通过信号转导上调VEGF表达,促进NSCLC组织中的血管生成;TNM分期和瘦素表达是影响NSCLC预后的独立风险因素。

【Abstract】 Objective To investigate the expressions of leptin,leptin receptor(OB-Rb) and vascular endothelial growth factor(VEGF) in tissue of non-small cell lung cancer,and their relationgship with the clinicopathological features and prognosis.Methods The immunohistochemistry PV-6000 was applied to detect the expressions of leptin,OB-Rb and VEGF in 30 NSCLC tissues and their adjacent normal lung tissues,the relationship of their expressions with the clinicopathological features and prognosis was analyzed by spearman rank correlation and COX regression.Results In NSCLC tissues,the positive rates of leptin,OB-Rb and VEGF expression were 70%,73.3% and 93.3%,respectively.The expressions of leptin,OB-Rb and VEGF in NSCLC tissues were significantly higher than that in adjacent normal lung tissues(P<0.05).The expressions of leptin,OB-Rb and VEGF had no correlation with gender,histological subtypes,lymph node metastasis or TNM staging in NSCLC tissues(P>0.05).There was a significant correlation between leptin and OB-Rb expression in NSCLC(P<0.05),there was also a significant correlation between OB-Rb and VEGF(P<0.05).TNM staging and leptin were independent prognostic factors.Conclusions The research indicates that after the binding of leptin and OB-Rb,the VEGF expression was up-regulated through signal transduction,thus promoting the vascular generation in NSCLC.Leptin and TNM staging are independent factors affecting the prognosis of NSCLC patients.
