脑干梗死的一种特殊MR征象分析

【摘要】 目的定义并探讨"脑干半切征"的MR表现特点、发病机制及其与脑干供血区分布的对应关系。方法回顾2009年1月至4月间山东省医学影像学研究所行颅脑MR检查的脑干病变患者的影像资料,分析"脑干半切征"的影像表现及发生机制,并探讨其解剖学基础。结果 286例脑干病变患者中,35例表现为病变不跨越脑干中线,且内缘与中线平齐,如刀切一样,即"脑干半切征",均位于一侧脑干的前内侧供血区,其中位于中脑1例(2.9%)、脑桥32例(91.4%),延髓2例(5.7%),经临床证实,35例患者均为脑梗死。结论 "脑干半切征"是脑干梗死的一种特殊征象,以脑桥多见,其影像学表现与脑干供血区解剖分布具有对应关系,可用于脑干病变的鉴别诊断。

【Abstract】 Objective To define the "brainstem hemi-section sign",and investigate its MR characteristics,pathogenesis and relation with vascular territories.Methods Clinical and conventional MRI data were collected from 286 patients with brainstem lesions from January to April 2009.MR findings,pathogenesis and anatomic basis of the "brainstem hemi-section sign" were analyzed.Results The "brainstem hemi-section sign" was found in 35 of 286 patients,and all lesions of these 35 cases were confirmed as brainstem infarction,showing a unilateral anteromedial brainstem infarct sharply marginated at the midline.All lesions in the 35 cases were located in unilateral anteromedial zones,1(2.9%) located at the midbrain,32(91.4%) located at the pons and 2(5.7%) located at the medulla oblongata.Conclusion The "brainstem hemi-section sign",consistent with branches and distribution of the brain stem artery,is special in brainstem infarction and mostly occurs in the pons.This sign plays an important role in the diagnosis of brainstem infarction and in the differential diagnosis of brainstem lesions.
