急性脑出血患者血清甲状腺激素水平的变化及其临床意义

【摘要】 目的探讨急性脑出血患者血清甲状腺激素水平及其临床意义。方法根据脑出血量多少、神经功能缺损程度及治疗2周后的疗效,将98例急性脑出血患者分为:脑出血量≤30ml为A组、>30ml为B组;神经功能缺损程度轻型的为C组、中型的为D组、重型的为E组;疗效为显效的为F组、有效的为G组、无效的为H组。选择40例健康者作对照。采用电化学发光法检测急性脑出血患者和健康对照者血清中T3、T4、FT3、FT4、TSH水平。结果急性脑出血患者A、B、C、D、E、G和H组与健康对照组,B组与A组,D、E组与C组,G、H组与F组之间,甲状腺激素水平差异均有统计学意义(P<0.05),T4、FT4、TSH水平升高,而T3、FT3水平下降。而疗效为显效的F组与健康对照组比较,甲状腺激素水平差异无统计学意义(P>0.05)。结论急性脑出血患者T4、FT4、TSH水平升高和T3、FT3水平下降与病情的严重程度及预后密切相关。

【Abstract】 Objective To study the clinical significance of thyroid hormone changes in the acute cerebral hemorrhage patients.Methods According to the different cerebral hemorrhage,the neurological deficit scores and prognosis after 2 weeks’ treatment,98 patients with acute cerebral hemorrhage were divided into≤30 ml group(A group) and >30 ml group(B group),or mild type group(C group),moderate type group(D group) and severe type group(E group),or excellence group(F group),effective group(G group) and ineffective group(H group).Serum thyroid hormones(T3,FT3,T4,FT4,TSH) were determined by electrochemiluminescence immunoassay(ECLIA) in 98 patients with acute cerebral hemorrhage and 40 controls.Results Compared with the control group,T3,FT3 of A,B,C,D,E,G and H group were markedly lowered and T4,FT4 and TSH were significantly increased in the acute cerebral hemorrhage patients(P<0.05),and there were no difference between thyroid hormone level of F group and control group(P>0.05).Compared with A,C or F group,T3,FT3 of B,D,E,G and H were markedly lowered and T4,FT4 and TSH were significantly increased(P<0.05).Conclusion T3,T4,FT3,FT4 and TSH levels were significantly correlated with the severity of acute cerebral hemorrhage.The determination of serum thyroid hormones would be useful in evaluating the severity of acute cerebral hemorrhage and the thyroid function of the disease.
