氯化铬-鞣酸法制备间接血凝试剂检测日本血吸虫抗体的研究

【摘要】 目的探索能提高检测日本血吸虫抗体的间接血凝诊断试剂(IHA)敏感性和特异性新的制备方法。方法分别以氯化铬法、氯化铬-鞣酸法制备间接血凝试剂,测定其敏感性与特异性,并与常规法相比较。结果对120份慢性血吸虫病人血清和96份正常人血清进行检测,常规法IHA试剂的敏感性度为93.3%,特异度为95 8%,约登指数0.891;氯化铬法IHA试剂的敏感度为85.8%,特异度为94 8%,约登指数为0.806:氯化铬一鞣酸法IHA试剂的敏感度为91.7%,特异度为97.9%,约登指数为0.896。在达到最优效价时,氯化铬-鞣酸法IHA试剂的可溶性虫卵抗原(SEA)用量为常规方法的一半。结论氯化铬-鞣酸法与常规法制备的IHA试剂相比较,其敏感性与特异性均较高,而其SEA致敏量则大为减少,节省了抗原,提高了工作效率。

【Abstract】 Objective To explore a new method for preparation of indirect hemagglutination assay(IHA) kit with high sensitivity and specificity for detection of Schistosoma japonicum antibody.Methods IHA kit was prepared according to chromium chloride method and chromium chloride-tannic acid method respectively, the sensitivity and the specificity of IHA kit prepared according to different methods were examined,and the results were compared.Results A total of 120 sera from patients with chronic schistosomiasis and 96 sera from healthy people were examined.The sensitivity,the specificity and the Youden index of routine IHA kit was 93.3%,95.8%and 0.891,respectively,while the values of the IHA kit prepared according to chromium chloride method was 85.8%,94.8%and 0.806,respectively,and the values of the IHA kit prepared according to chromium chloride-tannic acid method was 91.7%,97.9%and 0.896,respectively.The using dosage of the SEA of the IHA kit prepared according to chromium chloride-tannic acid method was the half of routine IHA kit when achieving the best effect.Conclusion Compared with routine IHA kit,the IHA kit prepared according to chromium chloride-tannic acid method has higher sensitivety and specificity in the diagnosis of schistosomiasis, and saves antigens because of the less using dosage of SEA.
