【摘要】 目的探讨枸橼酸体外抗凝在连续性血液净化治疗中的综合效果。方法选取2009年12月～2011年5月于本院进行连续性血液净化治疗的46例患者为研究对象,将其随机分为对照组(肝素组)23例和观察组(枸橼酸钠组)23例,后将两组的滤器使用时间、滤器及静脉壶凝血程度、滤器前后2、4、6、8h全血活化凝血时间、治疗前后血尿素氮、肌酐、Ca2+、pH、碱剩余、HCO3-、K+进行统计及比较。结果观察组的滤器使用时间长于对照组,滤器及静脉壶凝血程度优于对照组,滤器后2、4、6、8h全血活化凝血时间长于对照组,血尿素氮、肌酐低于对照组,P均<0.05,均有显著性差异,而治疗后两组的Ca2+、pH、碱剩余、HCO3-、K+均无显著性差异,P均>0.05。结论枸橼酸体外抗凝在连续性血液净化治疗中的综合效果较受肯定,安全性较高,综合优势优于肝素。更多还原

【Abstract】 Objective To study the comprehensive effect of extracorporeal citrate anticoagulation protocol in continuous blood purification.Methods 46 patients who were treated with continuous blood purification in our hospital from December 2009 to May 2011 were selected as research object,and they were randomly divided into control group(heparin group)23 cases and observation group(sodium citrate group)23 cases,then the using time of filter,coagulation degree of filter and vein pot,whole blood activated clotting time front and behind filter at 2,4,6 and 8 hours,blood urea nitrogen,creatinine,Ca2+,pH,base excess,HCO3-and K+ before and after the treatment were analyzed and compared.Results The using time of filter of observation group was longer than that of control group,coagulation degree of filter and vein pot were better than those of control group,whole blood activated clotting time front and behind filter at 2,4,6 and 8 hours were longer than those of control group,blood urea nitrogen,creatinine were lower than those of control group,all P<0.05,there were significant differences,but Ca2+,pH,base excess,HCO3-and K+ of two groups had no significant differences,all P>0.05.Conclusions The comprehensive effect of extracorporeal citrate anticoagulation protocol in continuous blood purification had positive effect,safety is higher,comprehensive advantage is better than heparin.更多还原
