右美托咪定对预防全身麻醉气管插管反应的影响

【摘要】 目的探讨右美托咪定对预防全身麻醉气管插管反应的影响。方法选择全身麻醉气管插管患者48例,随机分为对照组和右美托咪定(DXM)组,每组各24例。观察记录诱导前、插管前、插管即刻及插管后1、2、5min收缩压(SBP)、舒张压(DBP)、心率(HR)的变化。结果对照组患者诱导前与插管前SBP、DBP降低(P<0.05);插管即刻和插管后1、2 min时的SBP、DBP和HR均比诱导前升高,差异均有统计学意义(P<0.05)。DXM组患者诱导前与插管前SBP、DBP、HR明显降低,差异均有统计学意义(P<0.01);两组插管即刻和插管后1、2、5min与诱导前SBP、DBP水平比较,差异均无统计学意义(P>0.05),而HR比较,差异有统计学意义(P<0.01)。两组插管即刻和插管后1、2 min时的SBP、DBP和HR比较,差异均有统计学意义(P<0.05),两组插管后5 min时HR比较,差异有统计学意义(P<0.05)。结论 DXM可以很好的抑制气管插管的应激反应并减轻心血管反应,达到满意的麻醉深度。

【Abstract】 Objective To investigate the effects of dexmedetomidine in preventing adverse reactions induced by endotracheal intubation.Methods A total of 48 patients underwent endotracheal intubation were randomly divided into the control group and the dexmedetomidine(DXM) group(24 each).Systolic blood pressure(SBP),diastolic blood pressure(DBP) and heart rate(HR) were monitored and recorded before induction,before intubation,at intubation,1 min,2 min and 5 min after intubation.Results In the control group,SBP,DBP decreased both before induction and before intubation(P<0.05).SBP,DBP and HR increased at intubation,1 min and 2 min after intubation in comparison with before induction(P<0.05).In the DXM group,SBP,DBP and HR decreased both before induction and before intubation(P<0.01);SBP,DBP and HR at intubation and 1 min,2 min after intubation were significantly different between the two groups(P>0.05);HR of the two groups were significantly different 5 min after intubation(P<0.05).Conclusion Dexmedetomidine inhibits endotracheal intubation adverse reactions while reduces the cardiovascular response effectively.
