【摘要】 目的评价辖区内社康中心基本药物制度的实施情况及医生的处方行为,为制定社康中心运行、管理政策提供依据。方法采用横断面调查法,以调查表的方式收集全面实施基本药物制度前、后辖区内社康中心基本药物的配备、使用信息,并对医生的处方行为、合理用药情况进行分析。结果共有5家区属医院的10个社康中心接受了调查评估。全面实施基本药物制度后,区属医院管办的10家社康中心基本药物配备率为100.0%;基本药物让利金额增加156.4%;诊疗人次增加36.5%,每张处方平均金额总体下降14.5%;平均每张处方用药品种数小于5种,注射剂药物、抗菌药品使用比例以及合理用药处方比例有所增加。各社康中心上述指标存在差异。结论实施基本药物制度可切实减轻患者就医负担,行政措施的干预是确保基本药物制度及相关配套政策得以顺利实施的前提,对社区医生的处方行为进行规范和引导是提高基本药物使用率的关键。更多还原

【Abstract】 Objective To evaluate the implementation condition of national essential medicine system in the community healthcare centers in our district and doctors’ prescription behavior so as to provide comprehensive information for the policy-making on the operation and administration of the community health center.Methods The Cross section survey was used.The information of drug allocation and use in the community healthcare centers before and after comprehensive implementation of the national essential medicine system was collected by questionnaires.The condition of doctors′ prescription behavior and rational use of drugs was investigated by prescription evaluation.Results Ten community healthcare centers operated by 5 hospitals affiliated to the Health Bureau of Futian District involved in this investigation.After comprehensive implementation of the national essential medicine system,the percentage of essential drug allocation reached 100.0%.Compared to the data before comprehensively implementing the system,the discount percentage of price in essential drug increased by 156.4%;the percentage of visiting person-times increased by 36.5%;the average percentage of each prescription dropped by 14.5%.The average number of drugs in each prescription was less than 5.The utilization ratios of injection drugs and antimicrobial agents as well as the percentage of rationally prescribed drugs had increased significantly in all the community healthcare centers.Conclusion Comprehensive implementation of the national essential medicine system can definitely reduce the financial burden of patients.Interference of administrative measures is the essential prerequisite to guarantee smooth implementation of the system and its related policies.The key to improve the utilization of the essential medicines is to regulate and conduct the doctors′ behavior.更多还原
