环氧合酶-2在胃癌淋巴结转移灶中的表达及其与生存素和细胞核增殖抗原的关系

【摘要】 目的探讨环氧合酶-2(COX-2)在胃癌淋巴结转移灶中的表达及其与生存素(Survivin)、细胞核增殖抗原(Ki-67)表达的关系。方法对40例伴淋巴结转移的胃癌手术标本的原发灶、淋巴结转移灶进行COX-2、Survivin、Ki-67免疫组化染色,比较三者在淋巴结转移灶及原发灶中表达的差异。结果 (1)COX-2、Survivin在原发灶中强表达率分别为52.5%(21/40)、70.0%(28/40);在淋巴结转移灶中分别为72.5%(29/40)、72.5%(29/40)。COX-2在淋巴结转移灶中的表达显著高于原发灶,差异有统计学意义(χ2=4,P<0.05);但Survivin在淋巴结转移灶与原发灶中的表达强度比较差异无统计学意义(χ2=0.0667,P>0.05)。原发灶中Ki-67指数显著高于淋巴结转移灶(t=-2.17,P<0.05)。(2)双变量相关分析显示,原发灶中COX-2、Ki-67的表达与淋巴结转移灶中二者的表达均呈正相关(P<0.05)。(3)在原发灶中,COX-2与Survivin、Ki-67的表达均呈正相关(r值分别为0.6954和0.6084,P<0.01),Survivin与Ki-67的表达也呈正相关(r=0.6402,P<0.01);在淋巴结转移灶中,COX-2与Ki-67的表达呈正相关(r=0.3350,P<0.05)。结论胃癌淋巴结转移灶中COX-2表达与原发灶比较已经出现异质性变化;COX-2可能通过调控Survivin、Ki-67的表达而参与胃癌细胞的增殖和凋亡过程。

【Abstract】 Objective To investigate the expression of COX-2 in lymph node metastases(LNMs)of gastric carcinomas and its relationship to Survivin and Ki-67.Methods The expressions of COX-2,Survivin and Ki-67 were determined immunohistochemically in 40 paired primary tumors(PT) and LNMs of gastric carcinomas.Results The strong positive expression rates of COX-2 and Survivin were 52.5%,70.0%,respectively,in PT,and 72.5%,72.5%,respectively,in LNMs.The expression of COX-2 was higher in LNMs than in PT,the difference was significant(χ2=4,P<0.05).But no significant difference was noted in expression of Survivin between in PT and in LNMs(χ2=0.0667,P>0.05).The Ki-67 labeling index was higher in PT than in LNMs(t=-2.17,P<0.05).The expressions of COX-2 and Ki-67 in PT were positively correlated with those in LNMs(r=0.3484,0.5817,both P<0.05),COX-2 positively with Survivin and Ki-67(r=0.6954,0.6084,both P<0.01),and Survivin positively with Ki-67(r=0.6402,P<0.01);in LNMs,COX-2 positively with Ki-67(r=0.3350,P<0.05).Conclusion COX-2 expression in LNMs shows heterogeneity changes as compared with in PT.COX-2 may participate in cancer cell proliferation and apoptosis by regulating expressions of Survivin and Ki-67.
