【摘要】 目的探讨首发脑卒中患者急性期糖代谢异常的发生情况及其影响因素。方法采用前瞻性研究方法收集2009年6月—2010年6月唐山市工人医院神经内科收治的首发脑卒中患者287例,分析急性期糖代谢异常在年龄、卒中类型、既往病史等分布情况,采用多因素Logistic回归分析影响脑卒中患者急性期糖代谢异常的危险因素。结果女性、年龄≥65岁、出血性卒中、美国国立卫生研究院卒中量表(NIHSS)评分≥7分、高脂血症、体质指数≥30 kg/m2的脑卒中患者糖代谢异常的发生率分别高于男性、年龄<65岁、缺血性卒中、NIHSS评分<7分、无高脂血症、体质指数<30 kg/m2的脑卒中患者,差异均有统计学意义(P<0.05)。多因素Logistic回归分析表明:年龄、高脂血症、体质指数是脑卒中患者发生糖代谢异常的重要危险因素(P<0.05)。结论首发脑卒中患者急性期糖代谢异常的发生率较高,年龄、高脂血症和体质指数是糖代谢异常的重要危险因素。更多还原

【Abstract】 Objective To study the abnormal glucose metabolism in patients with first stroke in acute stage and its influencing factors.Methods By prospective study method,287 patients with first stroke admitted by the Department of Neurology,Tangshan Workers Hospital from June 2009 to June 2010 were collected.Distributions of abnormal glucose metabolism in age,stroke type,past medical history and others were analyzed.Multivariable Logistic regression analysis was applied to analyze risk factors influencing abnormal glucose metabolism in patients with acute stroke.Results In this study,the incidence of abnormal glucose metabolism in patients with age ≥65 years,hemorrhagic stroke,NIHSS score ≥7,hyperlipidemia and body mass index≥30 kg/m2 were significantly higher than those age <65 years,ischemic stroke,NIHSS score <7,no hyperlipidemia and body mass index <30 kg/m2,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that age,high cholesterol history,body mass index are important risk factors for abnormal glucose metabolism in patients with acute stroke(P<0.05).Conclusion The incidence of abnormal glucose metabolism in patients with acute stroke is high,and age,high cholesterol history,body mass index are the risk factors for abnormal glucose metabolism.更多还原
