口服葡萄糖-胰岛素释放试验者β细胞功能及胰岛素敏感性分析
Analysis of Insulin Sensitivity and β-cell Function of Subjects in OGIRT
【摘要】 目的观察145例口服葡萄糖-胰岛素释放试验(OG IRT)受试者的β细胞功能和胰岛素敏感性。方法分析145例受试者OG IRT结果,比较正常糖耐量组(NGT组)、葡萄糖调节异常组(IGR组)、2型糖尿病组(T2DM组)受试者的胰岛β细胞功能〔采用胰岛素分泌指数(△I30/△G30)和Homa胰岛素分泌指数(HOMA-β)评价〕及胰岛素敏感性〔采用Homa胰岛素抵抗指数(HOMA-IR)和胰岛素敏感指数(IAI)评价〕。结果 NGT组、IGR组、T2DM组FPG水平依次升高,ΔI30/ΔG30依次降低,组间比较差异均有统计学意义(P<0.01)。IGR组FINS显著高于NGT组(P<0.05)。NGT组、IGR组的HOMA-β显著高于T2DM组,组间比较差异均有统计学意义(P<0.01);IGR组、T2DM组的HOMA-IR显著高于NGT组,IAI显著低于NGT组,组间比较差异均有统计学意义(P均<0.01)。结论从NGT到IGR,胰岛素敏感性显著下降;从IGR到T2DM,胰岛β细胞功能显著受损。
【Abstract】 Objective To investigate insulin sensitivity and insulin secretion in three groups of normal glucose tolerance(NGT),impaired glucose regulation(IGT) and type 2 diabetes(T2DM) in OGIRT.Methods 73 subjects in NGT group,24 subjects in IGT group and 48 subjects in T2DM group were observed.Insulin secretion was evaluated by △I30/△G30 and HOMA-β.Insulin sensitivity was evaluated by HOMA-IR and IAI.Results Compared with NGT groups,FINS was significantly increased in IGR group(P<0.05).Three groups from NGT,IGR to T2DM,FPG level increased continuously,while △I30/△G30 level went down continuously(2.23±0.95,1.63±0.97 and 0.16±1.13 respectively,all P<0.01).Compared with T2DM group,HOMA-β was significantly increased in NGT and IGR group(P<0.01).Compared with NGT group,HOMA-IR was significantly increased in IGR and T2DM group,and IAI was significantly decreased in IGR and T2DM group(all P<0.01).Conclusion The transition from NGT to IGR,insulin resistance level rises continuously,and from IGR to T2DM,β-cell function goes down gradually.
