【摘要】 目的:探讨乳腺富含糖原透明细胞癌的临床病理特征与鉴别诊断特点。方法:观察7例乳腺富含糖原透明细胞癌的临床病理和免疫组化结果,并复习相关文献。结果:患者发病年龄42~75岁,中位年龄60岁。临床表现均为乳腺实性肿物。手术均为乳腺癌改良根治术。经随访12~27个月,2例出现肺脏转移。乳腺GRCC形态学特点:癌细胞为多角形,细胞边界清楚,可排列成巢团状、条索状或乳头状,超过90%的癌细胞胞质透明或细颗粒状,其内富含淀粉酶不稳定糖原,PAS染色(+)。免疫组化染色结果:CK(+),CEA、NSE、S-100蛋白、actin、vimentin均(-),约57%ER阳性,约42.9%PR阳性,约28.6%ER&PR阳性。结论:乳腺富含糖原透明细胞癌是一种少见的特殊类型乳腺癌,其诊断主要依靠组织病理学和免疫组化标识,生物学特征有待于进一步研究。乳腺GRCC治疗与乳腺非特殊类型浸润性癌治疗原则相同。更多还原

【Abstract】 Objective:To study the clinical pathologic characteristics and differential diagnosis of glycogen-rich clear cell carcinoma(GRCC)of breast.Methods: 7 cases of GRCC in breast were analyzed for correlation of clinicopathologic characteristics with immunohistochemical findings.Results: Patients median age was 60 years old(42～75yrs).All cases were shown solid lumps in the breast at diagnosis and treated with a modified radical mastectomy.Followed-up for 12～27moths.2 cases with distant metastasis in the lung.Histologically,GRCC cells of the breast were polygonal,smooth border and formed solid tumor nests,stringy or papillary structures,over 90% of the cancer cells cytoplasmic transparent or fine granular,which are rich in amylase unstable glycogen,PAS(+).Immunohistochemical stains results: CK(+),CEA(-),NSE(-),S-100 protein(-),actin(-),vimentin(-),about 57 percent ER(+),about 42.9% PR(+),about 28.6% ER&PR(+).Conclusion: Glycogen-rich clear cell carcinoma is a rare special carcinoma of breast,its diagnosis should mainly rely on histopathological and immunohistochemical logo,biology of GRCC needs to be studied further.GRCC of breast had the same treatable principle as the no-specially invasive cancer of breast.更多还原
