【摘要】 目的:探讨成熟NK细胞白血病的临床特点、诊断和治疗效果,以加深对该病的认识。方法:对6例成熟NK细胞白血病患者的临床资料进行回顾性分析。结果:侵袭性NK细胞白血病(ANKL)4例,慢性NK细胞白血病(CNKL)2例。4例ANKL病情呈暴发性,进展快,临床表现发热,肝脾肿大,贫血,多脏器功能衰竭,治疗无效死亡。2例CNKL病情呈慢性过程,表现为反复感染,淋巴细胞增多,1例给予甲氨蝶呤和糖皮质激素化疗后恢复正常。结论:成熟NK细胞白血病诊断主要依据临床表现、骨髓细胞学检查和免疫学分型,ANKL采用急性淋巴细胞白血病的化疗方案治疗。CNKL如有症状,给予以甲氨蝶呤为主的方案化疗。更多还原

【Abstract】 Objective:To explore the clinical characteristics,diagnosis and therapeutic effect of mature natural killer(NK) cell leukemia in order to enhance the recognition of the disease.Methods:Clinical data of 6 cases with mature NK cell leukemia were analyzed retrospectively.Results:The disease course was fulminant and developed rapidly in 4 cases with aggressive NK cell leukemia(ANKL) which presented with fever,hepatosplenomegaly,anemia and multiple organ failure.The therapeutic effect was poor and the patients died.Repeated infections and increased lymphocytosis were appeared in 2 cases with chronic NK cell leukemia(CNKL).One case recovered after receiving methotrexate and corticosteroid.Conclusions:The diagnosis of matural NK cell leukemia is based on clinical manifestations,bone marrow cytology and immunophenotyping.Chemotherapy of ANKL is as same as that of acute lymphoblastic leukemia.Treatment of CNKL is dominated by chemotherapy with methotrexate if there are symptoms.更多还原
