【摘要】 目的:探讨经单切口行邻近双节段后路显微内镜下椎间盘摘除术(MED)的可行性及其应用价值。方法:回顾我院2006年4月至2010年4月行双节段MED治疗的腰椎间盘突出症患者的临床资料,分为单切口和双切口组,应用MacNab评分标准进行术后疗效评估。选取我院2010年1月50名成年人的腰椎MRI资料,测量腰椎相邻两椎间隙中线在皮肤上交点的距离。结果:50名成年人腰椎相邻两椎间隙(L4/L5、L5/S1)中线在皮肤上交点的距离为(9.59±1.59)mm;单切口组为(13.14±5.44)mm、双切口组为(12.84±4.50)mm。经统计学处理两组数据组间差异无统计学意义,两组各与总体间分布差异也无统计学意义(P<0.05)。随访患者手术疗效单切口组优良率为88.9%,双切口组为91.7%。结论:经单切口行邻近双节段MED治疗腰椎间盘突出症可行有效。更多还原

【Abstract】 Objective: To explore the feasibility and application value of microendoscopic discectomy(MED) with single incision in treating double-segmental lumbar disc herniation.Methods: We retrospectively analysed the data of 21 cases of double-segmental lumbar disc herniation treated with MED from 2006 to 2010 in the Zhonga Hospital of Southeast University,the cases were divided into single-incision group and two-incision group,the effect were estimated by MacNab’s criteria.We also selected the lumbar spine MRI data of 50 adults in January 2010 in the Zhongda Hospital of Southeast University.We measured the distance between adjacent intersection of midline of the intervertebral space and the skin.Results:The distance between adjacent intersection of midline of the intervertebral space(L4/5 and L5/S1) and the skin was(9.59±1.59)mm,single-incision group was(13.14±5.44)mm and two-incision group was(12.84±4.50)mm.There was no significant difference between the two groups,and there was no significant difference between the each group and overall(P<0.05).88.9% patients in single-incision group gained excellent and good results,and 91.7% patients in double-incision group gained excellent and good results.Conclusion: MED with single incision in treating double-segmental lumbar disc herniation is feasible and effective.更多还原
