【摘要】 目的 ;探讨作者所在单位自1982年2月~2004年2月采用微血管减压术治疗三叉神经痛的技术要点分析。方法通过介绍22年来采用微血管减压术治疗1537例。作乳突后颅骨小切口,探查桥脑旁区,找到压迫神经的血管分开后用隔膜隔开减压。结果术后止痛者1496例(97.33%),死亡3例(0.19%),疼痛复发14例(0.91%)。并发症发生率约5.34%,发生小脑损害8例(0.52%),听神经损伤17例(1.1%),面神经损伤15例(0.98%),脊液漏15例(0.98%)。通过技术要点分析,近几年不断改进手术步骤中的技巧,扩大手术视野,减少手术并发症的发生。结论通过显微手术技术的总结,微血管减压术是治疗三叉神经痛安全且有效的治疗手段。更多还原

【Abstract】 Objective Microvascular decompression has become an accepted surgical technique for the treatment of trigeminal neuralgia.The authors began performing this procedure and has performed 1537 operations from February 1982 to February 2004.The purpose of this article is to review some of the nuances of the technical aspects of this procedure.Methods A review of 1537 operations shows that numerous modifications to the technique of microvascular decompression have occurred during the last 22 years.Mastoid skull post for a small incision,exploration pons adjacent areas,find the nerve blood vessels decompression with the membrane separating.Results The probability of becoming pain free after MVD in our study was 0.9733(1496 cases).There were 3 deaths(0.19%) and the incidence of serious complication was 5.34%.Our complication profile is similar to other authors.occurred 8 cases of cerebellar damage(0.52%),auditory nerve injury in 17 cases(1.1%),facial nerve injury in 15 cases(0.98%),15 cases of cerebrospinal fluid leakage(0.98%).The authors describe slight variations made to maximize surgical exposure and minimize potential complications in each of the several principal steps of this operation.These modifications have led to decreasing complication rates in recent years.Conclusion Using the techniques described in this report,microvascular decompression is an extremely safe and effective treatment for trigeminal neuralgia.更多还原
