FGFR3和HPA在膀胱移行细胞癌中的表达及其相关性研究

【摘要】 ①目的探讨成纤维细胞生长因子受体3(FGFR3)、乙酰肝素酶(HPA)表达与膀胱移行细胞癌(BTCC)生物学行为的关系。②方法膀胱移行细胞癌组67例标本经病理复检和临床分期、病理分级。11例正常膀胱组织标本作为对照组。应用免疫组化法检测组织FGFR3和HPA抗原表达情况。③结果 BTCC组FGFR3阳性表达率56.7%,对照组为阴性表达(P<0.01);Ta~T1肿瘤阳性表达率70.7%、T2~T4为34.6%,G1肿瘤阳性表达率79.1%、G2 68.2%、G3 19.0%,随肿瘤分期分级上升,阳性表达率下降(P<0.01);肿瘤初发和复发、年龄<65和年龄≥65岁、男性和女性组之间差异无统计学意义。BTCC组HPA阳性表达率64.2%,对照组阳性率为9.1%,差异有统计学意义(P<0.05);Ta~T1肿瘤阳性表达率53.6%,T2~T4为80.7%;G1肿瘤阳性表达率41.6%、G2 63.6%、G3 90.4%,随肿瘤分期分级上升,阳性表达率升高(P<0.05),肿瘤初发和复发组阳性表达率分别为54.5%和82.6%,差异有统计学意义(P<0.05),年龄<65和年龄≥65岁、男性和女性组之间差异无统计学意义。FGFR3和HPA表达成负相关(r=-0.464,P<0.01)。④结论 FGFR3、HPA与BTCC的发生发展、肿瘤的浸润转移密切相关,能较准确地评估BTCC的生物学行为,二者可作为BTCC有意义的肿瘤标志物。

【Abstract】 Objective To study the relativity of FGFR3 and HPA expression in 67 cases with bladder transitional cell carcinoma(BTCC).Methods The paraffin-embedded tissue sections from 67 patients with BTCC were reexamined.According to the UICC standard.41 cases were superficial turmors and 26 cases of invasive turmors.With the pathological diagnosive standard of WHO,there were G1 in 24 cases,G2 in 22cases and G3 in 21 cases.There were 44 cases with primary lesions and 23 cases with recurrent lesions.11 specimen obtained from normal bladder were used as controls.The expression of FGFR3 and HPA were detected by immunohistochemestry.Results Expression of FGFR3 was found in56.7%of BTCCs and negative in the control group,and70.7%of Ta~T1、34.6% of T2~T4.Approximately 79.1% of G1、68.2% of G2 and 19.0% of G3.With the progress of tumors clinical stage and grade,the positive rate of FGFR3 expression decreased(P<0.01).But there were no significant correlation between FGFR3 expression and postoperative reccurence.Expression of HPA was observed in 64.2% of BTCCs,and the controls showed 9.1%(P<0.05).It was also found in 53.6% of Ta~T1 and 80.7% of T2~T4,and 41.6% of G1,63.6% of G2 and 90.4% of G3.With the progress of tumors clinical stage and grade,the positive rate of HPA expression increased(P<0.05).There had significant correlation between HPA expression and postoperative recurrence(P<0.05).Furthermore,there was a significant inverse correlation between the expression of FGFR3 and HPA(r=-0.464,P<0.01).Conclusion FGFR3 and HPA play a key role in the oncogenesis and development of BTCC,they were closely related to the tumor invasion and metasis.FGFR3 and HPA could accurately evaluate the biological behaviour of BTCCs.They might act as useful tumor markers.
