【摘要】 目的回顾分析我院ICU病房2010年1-10月间临床分离菌对各种常见抗菌药物的耐药性。方法采用细菌培养、分离和鉴定以及K-B纸片扩散法,对分离菌进行药敏检测。结果临床分离202株细菌中,革兰阴性菌占71.8%,革兰阳性菌占17.8%。革兰阴性杆菌中,铜绿假单胞菌25株(12.4%),大肠埃希菌17株(8.4%),肺炎克雷伯菌38株(18.8%),鲍曼不动杆菌49株(24.3%)。革兰阳性球菌中,金黄色葡萄球菌19株(9.4%),肠球菌属3株(1.5%),凝固酶阴性葡萄球菌12株(9.9%)。金黄色葡萄球菌和凝固酶阴性葡萄球菌中耐甲氧西林株分别占89.5%和83.3%。未发现万古霉素耐药株。大肠埃希菌产ESBLs株检出率为70.6%,肺炎克雷伯菌中产ESBLs株检出率为78.9%。各种肠杆菌科细菌对亚胺培南、美洛培南仍高度敏感。结论甲氧西林耐药葡萄球菌(MRSA和MRCNS)、大肠埃希菌和肺炎克雷伯菌中产ESBLs菌株的检出率较高。定期进行耐药性监测有助于了解我院细菌耐药性变迁,为临床经验用药提供依据。更多还原

【Abstract】 Objective To investigate the antibiotic resistance of clinical isolates collected from intensive care unit of our hospital from Jan to Oct in 2010.Methods The bacteria were separated by bacterial culture and the drug resistance test was conducted using K-B method.Results In the 202 clinical isolates,gram negative bacilli and positive cocci accounted for 71.8% and 17.8% respectively.The most frequently isolated gram negative bacilli were P.aeruginosa(25,12.4%),E.coli(17,8.4%),Klebsiella spp(38,18.8%)and Acinetobacter.spp(49,24.3%).The most common gram positive cocci were S.aureus(19,9.4%),Enterococcus.spp(3,1.5%)and coagulase-negative staphylococcus.(12,9.9%).About 89.5% of S.aureus isolate were identified as methicillin-resistant Staphylococcus,83.3% of coagulase negative Staphylococcus isolates were methicillin-resistant.ESBLs-producing strain accounted for 70.6% in E.coli and 78.9% in Klebsiella spp.Isolates of Enterobacteriaceae were still highly sensitive to imipenem and meropenem.Conclusion The prevalence of MRSA and MRCNS is very high.ESBLs are prevalent in E.coli and Klebsiella spp.Periodic surveillance of bacterial resistance is most important and valuable for guidance of rational selection of antimicrobial agents.更多还原
