【摘要】 目的:探讨胰管结石合并慢性胰腺炎与胰腺癌的关系及其临床特点、诊断及治疗。方法:回顾性分析12例胰管结石合并胰腺癌患者的临床资料。结果:男性9例,女性3例。8例伴酒精性慢性胰腺炎,5例伴胆囊炎、胆管结石,3例合并糖尿病史,主要症状为上腹痛,腰背或双肩放射疼痛,脂肪性腹泻,三多一少,梗阻性黄疸。4例行胰十二指肠切除术;7例行胰体尾、脾脏切除、胰空肠Roux-en-Y吻合术。结论:胰管结石合并慢性胰腺炎与胰腺癌关系密切,可视为胰腺癌的癌前病变,且早期诊断困难,如病变性质难以确定,术中应取快速冰冻切片证实有无癌变。更多还原

【Abstract】 Objective:To investigate the relationship between pancreatic duct stone with chronic pancreatitis and pancreatic cancer,its clinical features,diagnosis,and treatment.Methods:Twelve cases of pancreatic duct stones associated with pancreatic cancer were analyzed retrospectively.Results:There were nine men and three women.8 cases had alcoholic chronic pancreatitis,5 cases had bile duct and cholecystitis,and 3 cases had history of diabetes.Chief symptoms included upper abdominal pain,back or shoulder radiation pain,fatty diarrhea and obstructive jaundice.4 patients were subjected to Pancreatoduodenectomy.7 patients underwent pancreatic body-tail resection,splenectomy and Pancreatic jejunal Roux-en-Y anastomosis.Conclusion:Pancreatic duct stones with chronic pancreatitis had close relationship to panceratic cancer,so it might be regarded as the precancerous lesion.Early detection is difficult.If it is hard to determine the nature of such lesion,intraoperative frozen section should be taken to confirm whether it is cancerous.更多还原
