原发性闭角型青光眼发病眼与对侧眼的生物解剖结构差异
Anatomical structure analysis of the patients with unilateral primary angle closure glaucoma
【摘要】 目的观察原发性闭角型青光眼(POAG)发病眼与对侧未发作眼(临床前期眼)解剖结构的差异。方法采用NIDEK Echoscan US-1800型A型超声测量系统对65例(130只眼)原发性急性闭角型青光眼(AACG)和38例(76只眼)原发性慢性闭角型青光眼(CACG)的解剖结构(前房深度、晶状体厚度、眼轴长度)进行测量,同时计算屈光度,使用SAS软件进行统计分析。结果 (1)AACG青光眼的发病眼和对侧未发作眼的解剖结构相比,屈光度、眼轴的均值有显著性差异。(2)AACG的发病眼和对侧未发作眼的解剖结构无显著性差异。(3)AACG的发病眼和CACG的发病眼相比,除前房深度外,各项均值均有显著性差异。且与对侧眼比较,除前房深度、晶状体深度外,各项均值有显著性差异。结论 AACG的患眼眼轴短、屈光度高。AACG与CACG的解剖结构存在差异,AACG的屈光度高、晶状体位置前移、眼轴更短。
【Abstract】 Objective To investigate the anatomical characteristics of the patients with unilateral primary angle closure glaucoma(PACG).Methods Anatomical structures including the anterior chamber depth(ACD), lens thickness(LT) and the axial length(AL),were measured by a contact ultrasound A-scan biometry device (NIDEK, Echoscan US-1800)in 130 eyes of 65 patients with acute angle-closure glaucoma (AACG), 76 eyes of 38 patients with chronic angle-closure glaucoma (CACG ). Refractive error measurements were obtained with an autorefractor (KR8800, Topcon, Tokyo, Japan).Statistic an- alysis was performed with SAS.Results There were significant differences in axial length and refractive error between the diseased eyes and the contralateral eyes without an onset in AACG group. There were no significant differences in all items in CACG group. Compared with the diseased eyes in CACG, the diseased eyes in AACG had significant differences in all items except ACD. There were significant differences in all items except ACD and LT between the diseased eye with AACG and CACG.Conclusion The patients with unilateral AACG have short axial length and high refractive error. There were significant differences between the AACG eyes and the CACG eyes in anatomical structure. AACG eyes had a high refractive error, shorter AL,and his lens position ahead.
