厄贝沙坦对充血性心力衰竭大鼠心肌间质MMP-2表达的干预研究

【摘要】 目的:探讨大鼠充血性心力衰竭心肌间质基质金属蛋白酶-2(MMP-2)的表达和心功能变化的关系及厄贝沙坦干预对此影响。方法:选用90只SD大鼠,8只不结扎冠状动脉列为假手术组(S组);其余82只结扎其左冠状动脉主干制成心肌梗死模型后,再分为对照组(C组)和厄贝沙干预组(E组)。应用超声心动图评价各组大鼠心功能变化,用免疫组化及Western免疫印迹方法检测各组大鼠MMP-2蛋白表达情况。结果:与S组相比,C组MMP-2的蛋白表达在实验第2周、4周、8周3个时点均明显升高(均P<0.05),而E组MMP-2蛋白表达在第4周和第8周明显降低(均P<0.05)。超声心动图结果显示C组、E组大鼠左室结构和功能呈现动态变化,心功能进行性降低,与C组相比,E组第8周心功能各项指标均有不同程度的改善。结论:大鼠心肌梗死后心肌间质内MMP-2的表达增高,是心室重塑和心力衰竭发生发展的机制之一。厄贝沙坦可通过抑制MMP-2的表达而改善心室重塑及心功能。

【Abstract】 Objective:To investigate the relationship of expression of matrix metalloproteinase-2(MMP-2) on ventricular remodeling and cardiac function following congestive heart failure(CHF) in the rats and the interventive effects of Irbesartan. Method:Ninety adult male Sprague-Dawley rats which underwent the left main coronary artery ligation were randomly devided into CHF control group(n=27) and CHF Irbesartan treatment group(n=25)(75 mg·kg-1·d-1).The rats in these two groups were divided into 3 sub-groups according to the course of treatment: 2-week,4-week,8-week sub-groups.Eight rats underwent sham operation,being killed after 8 week.Cardiac function and ventricular remodeling were evaluated by echocardiography.The protein expression of MMP-2 was examined by immunohistochemistry and western blot. Result:Compared to sham operated rats,the protein expression of MMP-2 were enhanced in all subgroups(P<0.05).In comparison with the CHF control group,the protein expression of MMP-2 was significantly lower in the 4 week and 8 week sub-groups in Irbesartan group(P<0.05). Conclusion:The expression of MMP-2 in the myocardial extracellular matrix increased after CHF.These changes suggested that MMP-2 played an important role in postinfarction extracellular matix remodeling and progressive CHF.Irbesartan can prevent ventricular remodeling and progressive heart failure by suppressing the enhanced expression of MMP-2.
