扁平苔藓的共聚焦激光扫描显微镜特征表现
The features of lichen planus in confocal laser scanning microscopy
【摘要】 目的:利用皮肤共聚焦激光扫描显微镜(CT)技术对扁平苔藓的皮肤CT特征进行研究。方法:对在我院就诊的33例临床诊断为扁平苔藓的患者,选定患者一处皮损先做激光共聚焦显微镜检查,后取该处皮损进行组织病理检查,并将二者作比较;对皮损附近的正常皮肤做激光共聚焦显微镜检查,与皮损进行对照。结果:皮损处皮肤CT的特征:明亮的角质层增厚;颗粒层细胞纵向扫描层次较正常处明显增多,细胞内强折光性颗粒较致密;棘层纵向扫描层次较正常处增多或减少。环状的真皮乳头结构模糊甚至消失,其间及真皮浅层可见致密的高折光度的淋巴细胞,此外真皮浅层还可见呈椭圆形或星形、边缘不整齐、多形态、强折光、大颗粒物质的噬黑素细胞。结论:结合临床表现,皮肤CT的图像特征可以为扁平苔藓的临床诊断及鉴别诊断提供有效的帮助。
【Abstract】 Objective:To evaluate the features of lichen planus in confocal laser scanning microscopy(CLSM).Methods:The lesions of 33 patients clinically diagnosed as lichen planus and the normal skin nearby lesions of them examined by CLSM,and then the biopsy was taken for histopathologic examination.And the results between the two methods were compared.Results:The characteristics of the lesions in the CLSM were:bright incrassate stratum corneum;obviously increased longitudinal scanning layers of granular cells than those in normal skin,with densely strong refraction particles in the cells;and increased or decreased longitudinal scanning layers of spinous cells in the epidermis.The annular structure of papillary dermis was blurred and even disappeared,with dense and highly refractive lymphocytes in the superficial dermis.Moreover,there were oval or stellate and melanophages looked like variably strong refractive particulate.Conclusion:Combined with clinical manifestations,CLSM can provide effective assistance for the clinical diagnosis and differential diagnosis of lichen planus.
