幼儿脑电双频指数与七氟醚呼气末浓度的关系

【摘要】 目的探讨幼儿患者脑电双频指数(BIS)与七氟醚呼气末浓度(CETSev)的关系。方法择期全麻下行腹部手术的患儿120例,ASAⅠ或Ⅱ级,年龄1~3岁,随机均分为四组:Ⅰ组,CETSev为0.4MAC;Ⅱ组,CETSev为0.7MAC;Ⅲ组,CETSev为1.0MAC;Ⅳ组,CETSev为1.3MAC。8%七氟醚面罩吸入诱导,氧流量5L/min,待患儿意识消失后调整氧流量至2L/min,并逐步调节七氟醚吸入浓度,按照分组使CETSev达到并维持于相应的浓度,每一浓度均维持15min,视为稳态CETSev。分别记录所有患儿入室(T0)、达稳态CETSev(T1)时的BIS、MAP及HR。结果当CETSev由0.4~1.3MAC递增时,四组患儿MAP和HR差异无统计学意义,但BIS逐渐下降(P<0.05),BIS、MAP及HR与CETSev相关系数分别为-0.857、-0.379和0(P<0.05)。结论无伤害性刺激时,BIS与CETSev呈一定剂量的负相关,相关性较好,BIS可用于监测七氟醚麻醉下的镇静程度。

【Abstract】 Objective To investigate the relationship between bispectral index(BIS)and end-tidal sevoflurane concentrations in young children.MethodsOne hundred and twenty ASA ⅠorⅡpediatric patients aged 1-3 y undergoing elective abdominal surgery under general anesthesia were randomized into four groups:group Ⅰ(end-tidal sevoflurane concentration of sevoflurane(CETSev)0.4 MAC);group Ⅱ(CETSev 0.7 MAC);group Ⅲ(CETSev 1.0 MAC);group Ⅳ(CETSev 1.3 MAC)Anesthesia was induced with 8% sevoflurane via facemask.Fresh gas flow(100% oxygen)was set at 5 L/min.The mask was manually sealed as tightly as possible.After loss of consciousness,oxygen flow was decreased to 2 L/min.The sevoflurane vaporizer was turned gradually so that CETSev were maintained at certain concentration.The changes of BIS,mean arterial blood pressure(MAP)and heart rate(HR)were recorded simultaneously at each time point.ResultsBIS decreased gradually without any significant changes in MAP and HR when end-tidal sevoflurane concentration increased from 0.4 MAC to 1.3 MAC(P<0.05).The correlation coefficient of BIS,MAP and HR with end-tidal sevoflurane concentration was-0.857,-0.379 and 0(P<0.05)respectively.ConclusionBIS correlates with end-tidal sevoflurane concentration and can be used to evaluate the depth of sevoflurane anesthesia in young children patients.
