高频及低频重复经颅磁刺激治疗抑郁症的对照研究
Control study on efficacy of high-and low-frequency transcranial magnetic stimulation in patients with depression
【摘要】 目的比较高频左侧与低频右侧两种重复经颅磁刺激刺激治疗方式治疗抑郁症的疗效。方法 80例非难治性抑郁症患者随机分2组,高频组采用10 Hz刺激左侧前额叶部位,低频组用1 Hz刺激右侧前额叶部位,每周治疗5次,进行为期3周的研究。采用汉密尔顿抑郁量表(HAMD24)、临床疗效总评量表(CGI)评定两种治疗方式的治疗效果,并记录不良反应。结果高频组有效率为52.8%(19/36),低频组有效率为47.5%(18/40),两组疗效比较差异无统计学意义(χ2=0.737,P>0.05)。两组患者HAMD24分值和CGI-SI分值较治疗前均有显著下降(P<0.01),但两组间比较差异无统计学意义(均P>0.05)。低频组不良反应率低于高频组(χ2=5.125,P<0.05)。结论低频右侧和高频左侧重复经颅磁刺激均能有效治疗抑郁症,两种治疗方式效果相当,低频右侧治疗方式耐受性更好,适应人群更广泛。
【Abstract】 Objective To compare the antidepressant efficacy between high-frequency on the left prefrontal cortex(HFL) and low-frequency on the right prefrontal cortex(LFR) in repetitive transcranial magnetic stimulation(rTMS) for the patients with depressive disorders.Methods 80 non-resistant depression patients were randomly divided into two groups for 3-week study,one group receiving high-frequency rTMS(10 Hz to stimulate the left prefrontal area),another group receiving low-frequency rTMS(1 Hz to the right prefrontal area).Hamilton Depression Scale(HAMD24) and Clinical Global Impression Scale(CGI) were used to assess the therapeutic effect of two treatment methods.Adverse reactions were observed and recorded in a 3 weeks’ duration.Results There was no significant difference in effective rate between high-frequency group(52.8%) and low-frequency group(47.5%) at study end point(χ2=0.737,P>0.05),there was a significant improvement in depressive symptomatology,as measured by scores on the HAMD24 and CGI-SI(P <0.01).The difference between two groups was not significant (P>0.05).The adverse reactions in low-frequency group were less than those in high-frequency(χ2=5.125,P<0.05).Conclusion High frequency rTMS on the left prefrontal and low frequency rTMS on the right prefrontal are similarly effective.Low-frequency rTMS is better tolerated,and it can extend application population with depression.
