【摘要】 目的探讨外周穿刺置入中心静脉导管(PICC)前应用超声确定颈外静脉分型的效果。方法拟进行颈外静脉PICC的患者(观察组)409例,穿刺前超声确定血管分型,测量血管内径、内膜、血液速度、动静脉距离,并观察周围淋巴结等情况;拟进行颈外静脉PICC的患者(对照组)386例,穿刺前常规肉眼观察、手触摸血管行PICC。结果与对照组比较,观察组颈外静脉一次置管成功率由89.9%提升至98.3%,合并症发生率静脉炎由6.0%降至1.2%,血栓由10.1%降至2.0%,导管异位率由3.9%降至0.5%,差异均有统计学意义(P<0.01)。结论根据血管超声确定颈外静脉分型后置入PICC,可有效地降低PICC合并症的发病率。更多还原

【Abstract】 Objective To explore the effect of ultrasound on external jugular vein typing before peripherally inserted central catheter(PICC) placement.Methods Four hundred and nine patients planned to perform PICC were in observation group,the internal diameter,intima,blood flow velocity of external jugular vein,arterious and venous distance,surrounding lymph nodes and other circumstances were measured by ultrasonography before puncture.Three hundred and eighty-six patients were in control group,PICC was performed by conventional macroscopic observation hand touching.Results Compared with control group,the achievement ratio of catheter placement in observation group was increased from 89.9% to 98.3%,the incidence rate of complications was decreased.phlebitis was from 6.0% to 1.2%,thrombi from 10.1% to 2.0%,ductal translocation rate from 3.9% to 0.5%.There was significant difference between two groups(P<0.01).Conclusion Typing of external jugular vein with ultrasound before PICC placement can efficiently decrease the occurrence of complications更多还原
