【摘要】 目的探讨老年人结直肠癌的临床特点、外科治疗、围手术期处理及术后并发症防治,以期提高手术疗效.方法回顾性分析2008年1月至2010年12月经外科治疗56例老年人结直肠癌患者的临床资料.结果全组病例均行手术治疗,术后随访1～28个月,随访率为100%,术后1 a生存率为85.71%(48/56),2a生存率为46.43%(26/56).38例患者术后出现并发症,其中肺部感染12例,切口感染5例,肠瘘3例,高血压3例,心功能不全6例,肝功能不全5例,急性肾功能衰竭2例,应激性溃疡2例,无死亡病例.结论老年人结直肠癌患者无特征性临床表现,且合并疾病较多,增加了手术风险.目前,手术治疗是老年人结直肠癌患者较好的治疗方法,加强围手术期处理,积极防治合并疾病,可提高患者的手术成功率、减少并发症、降低死亡率.更多还原

【Abstract】 Objective To explore the clinical characteristics,surgical treatment,perioperative management,and prevention of postoperative complications for the elderly patients with colorectal cancer so as to improve surgical curative effects.Methods The clinical data of 56 elderly patients with colorectal cancer from January 2008 to December 2010 were analyzed retrospectively.Results All the patients were treated surgically.56 patients were followed for 1 to 28 months and the follow-up rate was 100%.The survival rate was 85.71%(48/56)for 1 year and was 46.43%(26/56)for 2 years after surgical operation.38 patients had postoperative complications,including 12 cases of pulmonary infection,5 cases of infection of incisional wound,3 cases of intestinal fistula,3 cases of hypertension,6 cases of cardiac inadequacy,5 cases of hepatic inadequacy,2 cases of acute renal failure,and 2 cases of stress ulcer.No death was observed due to surgery.Conclusions The elderly patients with colorectal cancer have no characteristic clinical manifestations,but have many comorbidities,which increase the risk of surgical operation.However,the surgical operation is still a good treatment method for elderly patients with colorectal cancer.Strengthening perioperative management,and preventing and treating comorbidities actively can improve the surgical success rate,and reduce complications and mortality rate.更多还原
