右美托咪定对抑制经鼻气管插管反应的临床观察

【摘要】 目的了解右美托咪定在经鼻气管插管中对插管反应的抑制作用.方法 45例ASAI~II级18~55岁开口受限和行颈椎骨折内固定手术的患者,随机分为三组各15例,A组:表麻组;B组:表麻+右美托嘧定联合丙泊酚组;C组:表麻+丙泊酚组,行慢诱导经鼻气管插管,分别于入室、插管前、插管成功后5min、记录MBP、HR、SPO2、RR等指标.结果 B组在MBP、HR等指标与A组比较差异有统计学意义(P<0.05);在插管前时段C组RR与其它组比较差异有统计学意义(P<0.05).结论右美托咪定联合丙泊酚用于慢诱导经鼻气管插管,可以抑制插管引起的插管反应且无呼吸抑制.

【Abstract】 Objective To investigate the inhibitory effect of dexmedatomidine on the intubation response during nasal endotracheal intubation.Methods 45 patients(ASAI~II,18-55 years)undergoing cervical surgery or limited opening mouth were randomly divided into three groups:Group A,Group B and Group C(15 patients in each group).Patients in Group A received topical anesthesia only,patients in Group B received either topical anesthesia with propfol or dexmedetomidine,and patients in Group C received topical anesthesia and propfol during Slow-induced nasal endotracheal intubation.The mean arterial pressure(MAP),heart rate(HR),oxygen saturation(SpO2),and respiratory rate(RR)were recorded during loading,before induction and 5 minutes after tracheal intubation.Results There was a significant difference in MBP and HR between group A and group B(P<0.05).The was a significant difference in RR before induction between group C and other groups(P<0.05).Conclusion Combination of Dexmedetomidine with propfol during slow-induced nasal endotracheal intubation can decrease the cardiovascular responses during induction and has no significant respiratory depression.
