解脲脲原体对四环素的耐药性分析及其耐药机制研究
Prevalence and mechanism of antimicrobial resistance in tetracycline-resistant Ureaplasma urealyticum
【摘要】 目的了解解脲脲原体(Uu)对四环素的耐药性并分析其耐药机制。方法采用支原体培养、鉴定、药敏试验一体化试剂盒进行药敏试验;用PCR基因扩增方法对女性泌尿生殖道采集的200株Uu进行生物分群和耐药机制研究。结果 200株Uu中生物1群感染占70.0%(140/200),生物2群占22.0%(44/200)。生物1群对四环素的耐药率为18.6%(26株);生物2群对四环素的耐药率为20.5%(9株)。生物1群中耐四环素株的TetM基因阳性率88.5%(23/26);生物2群中9株耐四环素株中TetM基因阳性者8株。结论女性泌尿生殖道分离的Uu大多数为生物1群。四环素可以作为治疗Uu感染的一线药物,但耐四环素的Uu携带TetM基因的比例很高,TetM基因的检出率与生物种群无关,仍应根据药敏试验结果合理选用抗菌药物以延缓耐药株的产生。
【Abstract】 Objective To investigate the antimicrobial resistance and its mechanisms in tetracycline-resistant Ureaplasma urealyticum(Uu).MethodsThe Mycoplasma test kit of Lituo Company was used for identification,and susceptibility testing of Uu.The biotype of Uu and TetM gene were identified by polymerase chain reaction(PCR).ResultsA total of 200 samples were randomly selected for analysis.Most(70%,140/200) were positive for biotype 1 and 22%(44/200) were positive for biotype 2.TetM gene was identified in 23 of the 26 strains of tetracycline-resistant Uu biotype 1.TetM gene was positive in 8 of the 9 strains of tetracycline-resistant Uu biotype 2.ConclusionsBiotype 1 of Uu is one of the key pathogens causing Uu infections in women.Tetracycline can be used as the first-line antibiotic of choice to treat Uu infection.TetM gene is more prevalent in tetracycline-resistant Uu.The biotype of Uu is unrelated to TetM gene.Antimicrobial therapy should be rationally used in order to delay the emergence of resistant strain.
