糖尿病自身抗体检测在糖尿病分型中的诊断价值
The diagnosis value of detection of diabetes antibodies in the classification of diabetes mellitus
【摘要】 目的为糖尿病患者正确分型、早期预报、病因分析,提供实时诊断依据及与治疗方案。方法应用酶联免疫吸附法(ELISA)检测118例患者血清中抗胰岛素抗体(IAA-Ab)、抗胰岛细胞抗体(ICA-Ab)、抗谷氨酸脱羧酶抗体(GAD-Ab);同时用化学发光法检测患者血清中胰岛素水平。结果 118例糖尿病患者GAD-Ab阳性率为18.64%,ICA-Ab阳性率为9.32%,IAA-Ab阳性率为15.25%,联合GAD-Ab及ICA-Ab阳性率为24.4%。抗体阳性组患者的发病年龄、体质量指数、C肽水平低于阴性组患者,差异有统计学意义(P<0.05)。结论 应用ELISA检测患者血清中IAA-Ab、ICA-Ab、GAD-Ab,能为糖尿病患者正确分型、早期预报、病因分析提供诊断依据及治疗方案。
【Abstract】 Objective To provide the diagnosis basis and treatment protocols for the correct classification,early forecast,analysis of diabetes.Methods We used enzyme-linked immunosorbent method(ELISA) to test serum anti-insulin antibodies(IAA-Ab),islet cells antibody(ICA-Ab),glutamic acid decarboxylase antibody(GAD-Ab)of 118 patients,and the level of serum insulin of patients was tested by chemiluminescence method at the same time.Results The positive rates of GAD-Ab,ICA-Ab,IAA-Ab in 118 patients with diabetes were 18.64%,9.32%,15.25% respectively,and the positive rate of the joint detection of GAD-Ab and ICA-Ab was 24.4%.The pathogenesis of age,BMI,C-peptide levels in antibody positive groups were lower than those of the negative groups,with statistically significant difference(P<0.05).Conclusion It could provide the diagnosis basis and treatment protocols for the correct classification,early forecast,analysis of diabetes by testing the serum IAA-Ab,ICA-Ab,GAD-Ab using ELISA method.
