【摘要】 目的探讨异位促肾上腺皮质素(ACTH)综合征的临床特点、诊断及治疗。方法回顾2例异位ACTH综合征患者的诊断、治疗过程,复习相关文献,提出避免误诊、漏诊的措施。结果异位ACTH以糖代谢紊乱、高血压、低血钾、水肿等为临床表现;实验室检查表现为血浆皮质醇、ACTH明显增高及严重低血钾、碱中毒;CT可发现大部分肿瘤,手术切除是首选。结论异位ACTH综合征临床少见,其鉴别诊断困难、肿瘤定位重要,手术切除是最佳治疗方法。更多还原

【Abstract】 Objective To discuss the clinical features,diagnosis and treatment of ectopic ACTH syndrome.Methods Two cases of ectopic ACTH syndrome were presented and a literature review was conducted aiming to propose measures to avoid misdiagnosis and missed diagnosis.Results Abnormal glucose metabolism,hypertension,hypokalemia and edema of both lower limbs were the most common clinical symptoms.Laboratory examination showed significant increase in serum cortisol and ACTH,together with severe hypokalemia and alkalosis.Most of the primary lesions might be detected by CT.Resection of the primary lesion was the treatment of first choice.Conclusion The ectopic ACTH syndrome is very difficult to diagnose.Localization of tumor is of uttermost importance and resection of the primary lesions is the best treatment for ectopic ACTH syndrome.更多还原
