颈部动脉增强磁共振血管成像51例分析
Analysis on contrast-enhanced MRA of carotid artery in 51 patients
【摘要】 目的分析测试团块时间-信号曲线颈部动、静脉峰值差和不同的成像扫描方法对颈部动脉增强磁共振血管成像(CE-MRA)图像的影响。方法 51例患者接受颈部CE-MRA。对36例应用3D-FLASH序列(3D组),15例加用并行采集成像技术(iPAT,iPAT组)。对受检者均行2D-PC定位,在双侧颈动脉中心层面冠状位行3 ml/s小剂量(1~2 ml)Gd-DTPA测试团块。应用平均曲线对头臂静脉、上腔静脉、主动脉弓、颈总动脉、颈内静脉ROI绘制时间-信号强度曲线,测其峰值时间。分别评估图像动脉显影均匀度及静脉窦、颈内静脉、头臂静脉对观察动脉的影响。结果 51例受检者颈动脉测试峰值时间为(14.17±2.90)s,42例颈动、静脉峰值差为(7.95±2.12)s,36例上腔静脉(两次体循环)两次峰值时间间隔为(17.06±3.21)s。两组动脉显影均匀度差异无统计学意义,静脉窦显影对观察颈内动脉的影响两组差异有统计学意义(χ2=7.465,P<0.05)。结论颈部动脉CE-MRA中,应用测试团注,可获得动、静脉峰值时间和二次循环间隔时间,应用合适的成像序列和延迟时间可获得高质量的动脉血管图像。
【Abstract】 Objective To analyze the impact of carotid artery and venous enhancing peak time of time-intensity curves and different scanning methods on the image quality of contrast-enhanced magnetic resonance angiography(CE-MRA).Methods Totally 51 patients underwent CE-MRA of carotid,including 36 underwent 3D-FLASH(3D group) and 15 underwent 3D-FLASH and integrated parallel imaging techniques(iPAT group).All patients were located with 2D-PC,and the test bolus was initially performed with 3 ml/s(1—2 ml Gd-DTPA).ROI was drawn in the innominate vein,superior vena cava,aorta,common carotid artery and internal jugular vein,and the time-intensity curves were acquired according to the arteriovenous circulation time.The homogenesis of artery imaging was evaluated,as well as the impact of sinus venous,internal jugular vein and innominate vein on the artery imaging.Results The test peak time of 51 patients was(14.17±2.90)s,the difference of enhancing peak time between artery and veins in 42 patients was(7.95±2.12)s,the internal time of second circulation in 36 patients was(17.06±3.21)s.The homogenesis of artery image quality between the two groups were not statistically different.The impact of sinus venous on artery imaging showed significant difference(χ2=7.465,P<0.05).Conclusion Using test bolus,the enhancing peak time of artery,veins and internal time of second circulation can be acquired during carotid CE-MRA.Proper imaging protocol and delay time are helpful to obtaining high-quality artery images.
