【摘要】 目的:探讨进展期胃癌的CT表现及其对手术可切除性的术前评估价值。方法:分析100例经胃镜活检和/(或)手术病理证实的进展期胃癌的CT表现,评估手术切除的可行性,并与手术病理结果进行对照研究。结果:胃底贲门癌25例,胃体癌41例,胃窦癌19例,病变占据2个分区以上者15例。100例胃癌均显示胃壁有不同程度地增厚,部分胃壁有软组织肿块形成,黏膜面有溃疡形成,胃腔及贲门狭窄,贲门管壁增厚,食管下段受累以及周围组织器官侵犯等。CT对进展期胃癌的定位、定性诊断与胃镜活检和/(或)手术病理符合率高,肿瘤检出率可达100%。术前判断为可切除组的病例手术切除率达91.2%。结论:进展期胃癌术前CT诊断准确率较高,对肿瘤可切除性术前的评估有较高的参考价值。更多还原

【Abstract】 Objective:To discuss the performance of advanced gastric cancer of CT and surgical resectability of the value of preoperative assessment.Methods: To analyze CT manifestations of 100 cases advanced gastric cancer comfirmed by endoscopic biopsy and/ or pathology,explore the feasibility of surgery and contrast with the phathology.Results: There were 25 cases of fundus and cardia carcinoma,41 cases of corpus ventricular carcinoma and 19 cases of carcinoma of antrum; lesions in 15 cases involved more than two partitions.All cases showed increased thickness of gastric wall in various degrees,some of the cases showed soft-tissue masses in the gastric wall,ulcer of the mucosa,stricture of gastric cavity and cardia,increased thickness of cardiac wall,and invasion of lower esophagus and perigastric organs.Compared with gastroscopic biopsy and postoperative pathology, the location diagnosis and coincidence rate of advanced gastric carcinoma by CT was considerable,its detecting rate was 100%.The excision rate of the cases in the predicted operable group was 91.2% ; in the predicted unresectable group,the non-excision rate was 80.0%.Conclusion; It provided a high preoperative diagnostic accuracy of advanced gastric cancer with CT scanning and has a higher reference value in evaluating resectability of the tumor before surgery.It has important clinical significance.更多还原
