头颈部CTA在诊断脑血管性病变的诊断价值

【摘要】 目的探讨64排螺旋CT头颈部血管成像(CTA)对脑血管疾病诊断的价值。方法对临床诊为脑梗死(33例)、短暂性脑缺血发作(46例)、蛛网膜下腔出血(9例)、脑出血(5例)及脑血管畸形(3例)等共计96例脑血管疾病患者行头颈部CTA;应用容积再现、最大密度投影、多平面重组及高级血管分析进行分析、诊断。结果 73例头颈部血管显示单或多支、多段不同程序软硬斑形成致管腔不同程度狭窄及闭塞,血管变异5例,7例单侧椎动脉发育不良;12例动脉瘤或瘤样扩张,3例血管畸形。结论头颈部CTA对双侧颈内A、椎A、基底A、Willis环及大脑前、中、后动脉主干及其三级以上分支显示良好,对动脉软硬斑及管腔狭窄程度、血管变异、发育不良、动脉瘤及血管畸形等的诊断均有可靠的、重要的诊断价值。

【Abstract】 Objective To explore the clinical application value of 64-slice computed tomography angiography in cerebrovascular diseases.Methods 33 cases of cerebral infarction,46 cases of TIA,9 cases of subarachnoid hemorrhage and 5cases of cerebral hemorrhage,3cases of intracranial vascular malformations were examined with 64-slice spiral CTA.Images finding of cerebral vessels were evaluated by volume reconstruction maximum intensity projection,and multiplanar reconstruction.Results The 73 cases of patients were displayed single or multivessel,multisections soft -hard atheromatous plaque formation that result in angiostenosis or blockages,5cases of vascular variation,7 cases of single vertebral artery maldevelopment,12cases of aneurysma or aneurysmal dilatation,3 cases of vascular malformation.Conclusion The CT angiography in intracranial and cervical arteries can displayed the vessel in encephalic、vertebral artery、basal arteries、Willis circle、and precerebral artery、middle cerebral artery、posterior cerebral artery,including Level 3 above branch。It is dependable for diagnosis the Soft-hard atheromatous plaque、vascular variation、vertebral artery maldevelopment、aneurysma or aneurysmal dilatation、vascular malformation and Narrow degree of vessel。
