α-硫辛酸联合替米沙坦对2型糖尿病早期肾病患者蛋白尿及血管内皮功能的影响

【摘要】 目的分析α-硫辛酸和替米沙坦对2型糖尿病早期肾病患者尿蛋白排泄率及血管内皮功能的影响。方法将47例伴有微量白蛋白尿的2型糖尿病患者随机分为常规治疗组(例数=24)和联合治疗组(例数=23),两组均服用替米沙坦80mg,1次,d。联合治疗组予静脉注射α-硫辛酸600mg,1次/d,总疗程2周。治疗前后测定24h尿白蛋白排泄率(UAER)、血浆内皮素-1(ET-1)及血清一氧化氮(NO)水平。结果与治疗前比较,两组UAER均显著降低(P<0.01),联合治疗组降低更明显(P<0.05)。联合治疗组治疗后ET-1显著降低(P<0.01),NO明显升高(P<0.01);而常规治疗组ET-1、NO无明显变化(P>0.05)。结论α-硫辛酸联合替米沙坦能减少尿蛋白的排泄,改善血管内皮功能,对肾功能有一定的保护作用。

【Abstract】 Objective To investigate the effects of combined α-lipoic acid with telmisartan in the treatment of early diabetic neuropathy.Methods Totally 47 patients with early diabetic neuropathy were divided into two groups randomly:conventional treatment group(n=24) and combined treatment group n=23).Two groups were treated with telmisartan 80medday by oraladministration and combined treatment group was treated withα-lipoic acid 600mg/day by intravenous injection for 2 weeks.Urinary albumin excretion rate,Plasma level of nitricoxide(NO),endothelin-1(ET-1) were observed before and after treatment in two groups.Results After 2 weeks treatment,urinary albumin excretion rate was decreased obviously in both groups(P﹤0.01),but the efficacy in combined treatment group was better than that in conventional treatment group(P﹤0.05).In combined treatment group,the levels of NO were increased while the levels of ET-1 were decreased significantly.In conventional treatment group,there were no significantly changes in level of NO and ET-1(P>0.05).Conclusions This study suggested that combined α-Lipoic acid with telmisartan in treating early diabetic nephropathy can improve the endothelial function and decrease urinary albumin excretion rate obviously.
