急性脑梗死患者血清白介素-18、C-反应蛋白水平变化的研究

【摘要】 目的动态观察急性脑梗死患者血清白介素-18(Interleukin-18,IL-18)、C-反应蛋白(C-reactive protein,CRP)水平的变化,探讨炎症因子在脑梗死发病机制中的作用。方法抽取56例急性脑梗死患者发病1 d3、d和14 d时空腹静脉血,采用双抗体夹心酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)测定血清IL-18的水平,免疫速率散射比浊法测定血清CRP水平,并分别与35名健康对照者进行对比。并对急性脑梗死3 d时不同神经功能缺损程度的脑梗死患者IL-18和CRP水平进行对比。结果急性脑梗死患者发病1 d、3 d和14 d时血清IL-18、CRP水平均显著高于正常对照组(P<0.05),其中发病3 d内水平最高。急性脑梗死3 d时脑卒中神经缺损程度重度组中IL-18和CRP水平显著增高。结论急性脑梗死存在血清IL-18、CRP水平明显升高,提示炎症反应在急性脑梗死病理过程中起着重要作用,动态检测IL-18、CRP可作为病情评估和预后判定的敏感指标。

【Abstract】 Objective To explore the levels of IL-18 and CRP in the pathogenesis of acute cerebral infarction,and to discuss the role of cytokines in the pathogenesis of cerebral infarction.Methods This study included 56 patients with acute cerebral infarction and 35 healthy people as control.Serum IL-18 was measured by enzyme-linked immunosorbent assay(ELISA),and serum CRP was detected by the immune rate nephelometry.And the levels of IL-18 and CRP were compared with neurological deficits in varying degrees of cerebral infarction on the third day.Results Compared with control group,the serum levels of IL-18 and CRP on the first day,third day and 14th day in patients with acute cerebral infarction increased significantly(P<0.05),with the levels on the third day were the highest in patients with acute cerebral infarction.The serum levels of IL-18 and CRP were significantly higher of the severe neurological impairment group of stroke on the third day.Conclusion Serum IL-18 and CRP increase significantly in patients with acute cerebral infarction,suggesting that inflammatory reaction plays an important role in the pathological process of acute cerebral infarction.And dynamic detecting the serum levels of IL-18 and CRP can be used as sensitive indicators of assessment and disease prognosis.
