【摘要】 目的评价18F-FDG SPET/CT诊断食管癌淋巴结转移和病变长度的准确性。方法选择首程治疗行颈、胸、腹三野淋巴结清扫的胸段食管癌根治术患者40例,术前1周内行18F-FDG SPET/CT检查和食管X线钡餐检查,将18F-FDG SPET/CT诊断的食管癌转移淋巴结大小及病变长度与术后病理诊断及术中测量进行对比研究。结果 18F-FDG SPET/CT诊断食管癌总淋巴结转移的敏感性、特异性及准确性分别为44.4%、84.6%和37.1%,其中转移淋巴结短径<0.8 cm、0.8～1.2 cm和>1.2 cm的准确性分别为0%、43.8%和84.6%。术中测量与18F-FDG SPET/CT检测总的平均食管病变长度分别为(5.3±1.5)cm和(5.1±1.7)cm(P>0.05),其中pT1/2期平均长度分别为(5.4±1.9)cm和(5.0±2.0)cm(P>0.05),pT3/4期平均长度分别为(5.2±1.3)cm和(5.1±1.6)cm(P>0.05)。结论食管癌淋巴结转移大小明显影响18F-FDG SPET/CT诊断的准确性,转移淋巴结短径>1.2 cm时有较高的准确性;18F-FDG SPET/CT在诊断食管病变长度中也具有较高的准确性。更多还原

【Abstract】 Objective To evaluate the accuracy of 18F-FDG SPET/CT in the diagnoses of the metastatic lymph nodes and the lesion length of esophageal carcinoma. MethodsForty patients with esophageal carcinoma in thoracic segments received initial treatment with 3-fleld lymphadenectomy were enrolled.All the cases received 18F-FDG SPET/CT and X-ray barium meal in one week before operation.The metastatic lymph nodes and the lesion length diagnosed with 18F-FDG SPET/CT were compared with the intraoperative measure and the postoperative pathological results. Results18F-FDG SPET/CT analyses in total metastatic lymph nodes showed the sensitivity of 44.4%,the specificity of 84.6% and the accuracy of 37.1%.The accuracy was 0%,43.8% and 84.6% for the metastatic lymph nodes with the shorter diameters<0.8 cm,0.8-1.2 cm and>1.2 cm,respectively.The total lesion lengths were(5.3±1.5) cm of the intraoperative measure and(5.1±1.7) cm of 18F-FDG SPET/CT(P>0.05);and they were(5.4±1.9) cm and(5.0±2.0) cm for pT1/2 stage(P>0.05),and were(5.2±1.3) cm and(5.1±1.6) cm for pT3/4 stage(P>0.05).
