不同程度缺氧OSAHS患者前庭脊髓反射的观察
Effects of different degree hypoxia on the vestibulospinal reflexes in patients with obstructive sleep apnea and hypoventilation syndrome
【摘要】 目的对不同程度缺氧阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者进行静态姿势描记(PSG),观察不同程度缺氧对OSAHS患者前庭脊髓反射的影响。方法根据缺氧程度不同将98例OS-AHS患者分为3组:轻度组31例、中度组34例、重度组33例,选30例为对照组,分别行PSG,对33例重度组OSAHS患者行持续正压通气治疗(CPAP)前后PSG测试对比。结果 PSG各参数轻度组比较差异无统计学意义(P>0.05),中度组与对照组、轻度组比较差异有统计学意义(P<0.05),重度组与对照组、中度组、轻度组比较差异有统计学意义(P<0.01);OSAHS患者CPAP治疗3个月外周面积及路径总长较治疗前降低(P<0.05),其中晃动速度减低更为显著(P<0.01)。结论中、重度OSAHS患者的前庭脊髓反射功能下降,CPAP有助于改善OSAHS患者的前庭脊髓反射功能。
【Abstract】 Objective To observe the effects of different degree hypoxia on the vestibulospinal reflexes(VSR) in patients with obstructive sleep apnea and hypoventilation syndrome(OSAHS) by static posturography(PSG).Methods A total of 98 patients with OSAHS were divided into 3 groups according to the degree of hypoxia,which were mild group(group B,n=31),moderate group(group C,n=34),severe group(group D,n=33),and 30 volunteers were enrolled as control group(group A).The parameters of PSG were measured respectively.The PSG parameters for 33 patients in group D were compared before and after treatment by continuous positive airway pressure(CPAP).Results There were no significant differences in these parameters of static PSG between group B and group A(P>0.05),however,there were significant differences between group C and group A,group B(P<0.05),and there were significant differences between group D and the other three groups(P<0.01).After 3-month treatment by CPAP,the periphery area and total pathway length in 33 patients with severe OSAHS were significantly decreased,as compared with those before treatment(P<0.05),especially,the sway velocity was reduced much more obviously(P<0.01).Conclusion The VSR function in patients with mild or severe OSAHS is decreased,and CPAP contributes to improving the VSR function in patients with OSAHS.
