【摘要】 目的探讨Fogarty球囊导管在尿毒症患者自体血管动静脉内瘘成形术及取栓术中的应用。方法对2006年1月至2009年12月我科室收治52例尿毒症患者,18例行自体血管动静脉内瘘成形术,34例因静脉血栓形成行自体血管动静脉内瘘取栓术。术中使用Fogarty导管,在球囊充盈状态下牵拉出血管,达到扩张细小血管或取栓目的,对治疗效果进行总结评价。结果 52例患者中通过使用Fogarty球囊导管,手术成功,无并发症,术后内瘘使用通畅,血液透析时血流量达200ml/min以上。随访12个月,其中6例行动静脉内瘘取栓术患者再次出现血栓形成。结论 Fogarty球囊导管在尿毒症自体血管条件欠佳患者中行动静脉内瘘成形术、在动静脉内瘘血栓形成患者中行取栓术是有效治疗方法。更多还原

【Abstract】 Objective To evaluate the effect of Fogarty balloon catheters in uremic arterial-venous fistula operation and thrombus removal.Methods 52 uremic patients were selected between January 2006 and December 2009,Fogarty balloon catheters were applied in 18 cases of arterial-venous fistula operation and 34 cases of arterial-venous fistula thrombus removal.In operation patients,Fogarty balloon catheters were put inside the small vessels,after saline infusion,the balloon were pulled out intending to dilate the vessels.In fistula thrombus removal,after the Fogarty balloon catheters getting through the thrombus and balloon saline infusion,catheters were pulled out.Results All cases of arterial-venous fistula operation and thrombus removal were successfully done and no complications were seen.All fistulas were successfully used and the blood flow reached over 200 ml/min.After a 12 months followed-up,6 cases of arterial-venous fistula thrombi recurred.Conclusion Using Fogarty balloon catheters in uremic arterial-venous fistula operation and thrombus removal is a safe and effective way in uremic patients with small operating vessels and arterial-venous fistula thrombus.更多还原
