经冠状动脉病变血管植入体外培养扩增的自体骨髓间充质干细胞对急性心肌梗死区的疗效研究
Effect of Transplantation of Bone Marrow Derived Mesenchymal Stem Cells through Coronary Artery Lesion into Post-infarct Myocardium
【摘要】 目的探讨经冠状动脉病变血管植入体外培养扩增的自体骨髓间充质干细胞(MSCs)对急性心肌梗死区的影响。方法体外扩增培养巴马香猪MSCs。用球囊扩张封堵冠状动脉前降支制备巴马香猪急性心肌梗死模型,成模1个月后,经冠状动脉前降支植入体外扩增培养的自体MSCs至急性心肌梗死区,观察其对心肌梗死区的影响。结果梯度离心法分离结合贴壁筛选法体外扩增培养的巴马香猪MSCs第3代细胞表面标记物CD71阳性率为95%,CD34的阳性率为0.0%。传代培养4代后,细胞开始衰老。经冠脉病变血管球囊导管法植入体外培养扩增的自体MSCs后4周,MSCs组梗死区可见BrdU阳性植入的MSCs,对照组梗死区未见BrdU表达阳性细胞;MSCs组射血分数、舒张末期室间隔厚度显著改善(P<0.05),梗死面积显著减少(P<0.05),新生血管数多于对照组(P<0.05)。两组均未见横纹结构。结论 经冠状动脉病变血管导管法植入体外培养扩增的自体MSCs治疗急性心肌梗死是有希望的新方法,值得深入研究。
【Abstract】 Objective To investigate the therapeutic effects of transplantation of bone marrow mesenchymal stem cells(MSCs)with over-the-wire balloon angioplasty catheters through coronary artery lesion into post-infarct myocardium in Bama Xiang pigs.Methods MSCs were isolated and cultured in vitro.The acute myocardial infarction(AMI)models were induced by balloon blockage of left anterior descending coronary artery in Bama miniature pigs.Purified MSCs were autologously transplanted into target post-infarct myocardium by intra-coronary artery one month after AMI and the effects were observed.Results CD71 expression of the third passage of MSCs after purification and expansion was 95%while CD34 was 0.0% by flow cytometry detection.After 4 passages,MSCs began senescing.Four weeks after transplantation,BrdU-positive MSCs could be seen in MSCs group.The cardiac functions in the MSCs group improved more significantly than those in the control group and interventricular septal thickness at end diastole(P<0.05).The size of the myocardial infarction in MSCs group was smaller than that in control group(P<0.05).The number of regenerate capillary in MSCs group was more than that in control group(P<0.05).No rhythmic structure was seen in both groups.Conclusion Transplantation of MSCs through coronary artery lesion by over-the-wire balloon catheter can improve the cardiac function and decrease the size of myocardial infarction,therefore it may be beneficial and hopeful in the treatment of AMI and worth further study.
