【摘要】 目的探讨抗核抗体(ANA)、免疫球蛋白和补体检测对狼疮性肾炎(LN)和不伴肾炎的系统性红斑狼疮(SLE)的诊断意义。方法对406例SLE患者和120例健康体检者采用间接免疫荧光法测定ANA,应用散射比浊法测定补体(C3、C4)及免疫球蛋白(IgG、IgA、IgM、IgE)。结果 406例SLE患者中,ANA阳性率94.49%,与对照组比较,差异有统计学意义(P<0.01)。SLE患者LN组与非LN组ANA核型均以核均质型和核颗粒型为主,LN组这2种核型占84.35%,非LN组占72.12%,两者差异有统计学意义(P<0.05);LN组胞浆颗粒型占11.31%,与非LN组的21.93%差异有统计学意义(P<0.05)。LN组与非LN组IgG、IgA、IgM、IgE水平均增高,而C3、C4水平均降低,与对照组比较差异有统计学意义(P<0.05);C3水平在LN组降低更明显,与非LN组比较差异有统计学意义(P=0.001)。结论 ANA、免疫球蛋白和补体的联合检测对提高SLE的临床诊断、预后判断、疗效观察等方面意义重大。更多还原

【Abstract】 Objective To explore the diagnostic significance of combined detection of antinuclear antibody(ANA),immunoglobulin and complement for systemic lupus erythematosus(SLE)with or without lupus nephritis(LN).Methods ANA was detected by indirect immunity fluorescent method,and complement(C3,C4)and immunoglobulin(IgG,IgA,IgM,IgE) were detected by nephelometry in 406 SLE patients and 120 health controls.Results Among 406 SLE patients,the positive rate of ANA was 94.49%,and there was significant difference with control group(P<0.01).Most SLE patients were with homogeneous and speckled types of ANA.These two types were 84.35% in LN group,higher than that of 72.12% in without LN group(P<0.05).Cytoplasmic type was 11.31% in LN group,lower than that of 21.93% in without LN group(P<0.05).Among all patients,the serum levels of IgG,IgA,IgM and IgE were all increased,but serum levels of C3 and C4 were lower than control group(P<0.05).Serum level of C3 in LN group were significant lower than that in without LN group(P=0.001).Conclusion Combined detection of ANA、immunoglobulin and complement have certain clinical value of the diagnosis,prognostic evaluation and rational adjuvant-therapy of SLE.更多还原
