隐球菌性脑膜炎及病毒性脑炎患者脑脊液IL-6、IL-10和TNF-α含量检测及其临床意义

【摘要】 目的:探讨隐球菌性脑膜炎及病毒性脑炎患者脑脊液中白细胞介素-6(IL-6)、IL-10和肿瘤坏死因子-α(TNF-α)含量的变化及临床意义。方法:应用Luminex液相芯片技术对隐球菌性脑膜炎患者18例(隐脑组),病毒性脑炎患者20例(病脑组)及非中枢神经系统感染患者15例(对照组)脑脊液中IL-6I、L-10和TNF-α进行检测、比较;同时分析隐脑组患者脑脊液中3种细胞因子之间的相关性。结果:隐脑组脑脊液中IL-6水平高于病脑组(P<0.01)和对照组(P<0.05),IL-10水平较对照组显著升高(P<0.001),TNF-α水平较病脑组(P<0.05)和对照组(P<0.001)升高;病脑组脑脊液IL-6I、L-10和TNF-α水平较对照组增高(P<0.001,P<0.001,P<0.05)。隐脑组IL-6与IL-10呈显著正相关(r=0.50,P<0.05);IL-6与TNF-α之间未见显著相关性;IL-10与TNF-α之间亦未见显著相关性。结论:IL-6I、L-10和TNF-α可能参与隐球菌性脑膜炎和病毒性脑炎的病理生理过程,其含量变化对两种疾病鉴别诊断的临床参考价值有待更多研究。

【Abstract】 Objective: To investigate the concentrations of interleukin-6(IL-6),IL-10 and tumor necrosis factor-α(TNF-α) in cerebrospinal fluid(CSF) of patients with cryptococcal meningitis or viral encephalitis and to analyze the clinical implication of the cases.Methods: Eighteen patients with cryptococcal meningitis(CM group),twenty with viral encephalitis(VE group) and fifteen control volunteers without any central nervous system infection(control group) were enrolled in the study.CSF samples were obtained at admission and the concentrations of IL-6,IL-10 and TNF-α were identified using the Luminex xMAP fluid microsphere-based array system.Results: The concentrations of IL-6 and TNF-α in CM group were significantly higher than those in VE(P<0.01) and control groups(P<0.05).The concentration of IL-10 was higher in CM group than that in the control group(P<0.001).In addition,the concentrations of IL-6,IL-10 and TNF-α were significantly increased in VE group compared with those in the control group(P<0.001;P<0.001;P<0.05).Correlation analysis showed that IL-6 was positively related to IL-10 in CM group(r=0.50,P<0.05).Conclusion: IL-6,IL-10 and TNF-α may play important roles in pathophysiological process of cryptococcal meningitis and viral encephalitis.More studies are needed to illustrate the association of prognosis and the changes in the cytokines.
