血清免疫球蛋白、轻链测定对多发性骨髓瘤诊断的临床价值

【摘要】 目的:探讨血清免疫球蛋白(Ig)、轻链(κ、λ)的含量测定对多发性骨髓瘤(MM)诊断的临床价值。方法:速率散射比浊法。结果:39例MM的初诊患者,Ig测定结果显示,IgG型27例,占69.2%,IgG含量(60.19±29.80)g/L;IgA型7例,占17.9%,IgA含量(38.10±20.72)g/L;IgM型1例,占2.6%,IgM含量39.40g/L;轻链型4例,占10.3%,其IgG、IgA、IgM含量减低,单一类型Ig含量升高的MM组与其他MM组Ig含量相比有显著性差异(P<0.01),与正常对照组相比有显著性差异(P<0.01)。39例MM血清轻链含量都有单一类型升高,轻链κ含量升高为21例,占53.8%,λ含量升高为18例,占46.2%。κ型MM组、λ型MM组同一类型之间κ或λ含量与正常对照组相比有显著性差异(P<0.01)。结论:血清Ig、轻链κ、λ含量可作为MM的诊断和分型的重要指标,其检测结果对MM诊断及鉴别诊断提供了可靠的依据,具有重要的临床价值。

【Abstract】 Objective To explore the clinical value of serum immunoglobin(Ig) and light chain(κ,λ) measurement on the diagnosis of multiple myeloma(MM).Methods Rate dispersion turbidimetric method.Results In 39 cases of MM patients,IgG type covered 27(69.2%),whose IgG concentration was 60.19±29.80g/L,IgA type covered 7(17.9%),whose IgA concentration was 38.10±20.72g/L,IgM type covered 1(2.6%),whose IgM concentration was 39.4g/L.Light chain type covered 4(10.3%),whose IgG IgA and IgM reduced.Dfferent groups of MM’s Ig had significance difference(P1<0.01).And their Ig had evident difference with normal control(P2<0.01).Single type of light chain increased in 39 cases of MM,high κ concentration covered 21(53.8%),and high λ concentration covered 18(46.2%).Serum κ or λ concentration in the same type of κ type of MM and λ type of MM had significant difference with normal control(P<0.01).Conclusion Serum concentration of Ig,κ,λ can act as important index of MM’s diagnosis and classification.Their results provide dependable evidence for the diagnosis and differential diagnosis of MM,which have important clinical value.
