【摘要】 目的:探讨急性心肌梗死(AM I)患者血浆N末端脑钠肽原(NT-proBNP)水平的变化及其与心功能指标的相关性。方法:采用荧光免疫分析对189例AM I患者及55例健康对照者进行血浆NT-proBNP测定,同时以彩色多普勒超声心电动仪测定AM I患者左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF),并与血浆NT-proBNP水平进行相关性分析。结果:AM I患者血浆NT-proBNP水平明显高于健康对照者(P<0.01),且随着患者心功能受损程度的加重(K illip心功能分级的升高)而梯次增高,致使不同K illip心功能分级患者之间的血浆NT-proBNP含量也呈现出显著差异(P<0.01);AM I患者血浆NT-proBNP水平与LVEF、LVESD、LVEDD呈现良好的相关性(r分别为-0.39、+0.36和+0.41,P均<0.01)。结论:AM I患者血浆NT-proBNP水平与LVESD、LVEDD呈正相关,与LVEF呈负相关,是评价心室功能的灵敏指标。更多还原

【Abstract】 Objective To explore the changes of the plasma levels of N-terminal pro brain natriuretic peptide(NT-proBNP) and the correlation between the plasma levels of NT-proBNP and the parameters of ventricular function in patients with acute myocardial infarction(AMI).Methods The plasma levels of NT-proBNP were measured by fluorescence immunoassay in 189 patients with AMI and 55 healthy subjects.Left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD)were determined by color Doppler ultrasonograghy in AMI patients.And the correlation between the plasma levels of NT-proBNP and the values of LVEF,LVESD and LVEDD was studied in AMI patients.Results The plasma levels of NT-proBNP were markedly elevated in patients with AMI as compared with healthy subjects(P<0.01).As the plasma levels of NT-proBNP were increased in proportion to increasing Killip grades(Ⅰ～Ⅳ),there were significant differences in the plasma levels of NT-proBNP in AMI patients from different groups divided by Killip classification(P<0.01).There was obvious correlation between the plasma levels of NT-proBNP and the values of LVEF,LVESD and LVEDD(r was-0.39,+0.36 and +0.41 respectively,P<0.01).Conclusion The plasma level of NT-proBNP was positively correlated with LVESD,LVEDD,negatively correlated with LVEF and could serve as a sensitive parameter for evaluating ventricular function in patients with AMI.更多还原
