2009年山东省克山病病情调查结果分析
Analysis of Keshan disease in Shandong Province in 2009
【摘要】 目的观察山东省克山病病情,掌握克山病的发病情况,科学指导防治科研工作的开展。方法按照全国克山病调查方案要求,2009年在克山病病区设立调查点,每个点对500名左右自然人群进行体格检查、心电图描记,可疑心脏异常者行后前位X线胸部摄片,同时调查内外环境硒含量、居民经济收入及人均占有粮食等情况。结果 8个调查点共调查居民3 840人,检出克山病患者144例(其中潜在型140例,慢型4例),检出率为3.75%。异常心电图检出率为19.6%,其中以T波改变、ST-T改变、窦性心动过缓、QRS波群低电压、ST改变、房性早搏等多见。拍摄X线胸片102例4,3例增大(其中轻度增大26例,中度增大11例,重度增大6例),异常检出率为42.2%。病区居民发硒含量接近硒适宜地区的水平,外环境粮食硒含量仍处于较低水平。结论山东省克山病病区居民病情处于稳定态势,内环境硒水平的升高与当地居民生活水平的提高,膳食营养的改善有关。
【Abstract】 Objective To observe the condition and control the incidence of Keshan disease(KSD) in Shandong Province and supply evidence for scientific control and study.Methods According to the scheme of survey condition of KSD in China,the survey fields of KSD were set up in Shandong KSD areas.In each point,about 500 natural populations were checked by physical examination,electrocardiography recording.The suspicious people were shooten chest X-ray film.At the same time,selenium inside and outside environment was determined and life condition of inhabitant was surveyed.Results In 8 investigation points,3 840 residents were surveyed and 144 cases of Keshan disease were detected(in which 140 cases of Potential,4 cases of Chronic),the detection rate was 3.75%.The incidence of abnormal electrocardiogram(ECG) was 19.6%,of which T-wave change,ST-T changes,sinus bradycardia,QRS-wave group of low-voltage,ST changes,atrial premature beats,etc,were more common.Of the 102 X-ray films,43 cases have enlarged heart(in which 26 of mild,11 of moderate,6 of severe) and the abnormal detection rate was 42.2%.The selenium content of hair was approaching the level of appropriate areas,and the external environment selenium content of food was still lower.Conclusions The current condition of KSD in Shandong Province remains relatively stable.Selenium levels in the internal environment were elevating because of the local living standards and dietary nutrition improvement.
