异丙酚联合芬太尼静脉麻醉在人工流产术中的应用与护理

【摘要】 目的分析异丙酚联合芬太尼静脉麻醉在人工流产术中的应用效果,寻找适合的综合护理措施。方法选取2009年8月~2010年5月收治的施行人工流产术孕妇90例,按照随机原则分为研究组(A组,n=45例)和对照组(B组,n=45例),研究组人工流产术孕妇给予静脉推注芬太尼(1~2μg/kg)和异丙酚(2mg/kg);对照组单纯使用异丙酚静脉注射(2mg/kg),观察比较两组的麻醉效果以及宫颈松弛程度、遗忘程度、术中术后情况及平均动脉压(MAP)、心率(HR)、呼吸(R)、血氧饱和度(SpO2)等情况。同时总结所实施的相关综合护理措施的经验体会。结果两组孕妇镇痛效果均较满意,研究组与对照组比较发现,在宫口松弛、遗忘程度、手术时间方面研究组明显优于对照组(P<0.05)。结论异丙酚联合芬太尼用于人工流产术中效果满意,发挥镇痛作用同时,不良反应发生少,安全有效,值得临床推广应用。在实施护理措施的同时需注意观察呼吸、循环监护及术后的麻醉反应。

【Abstract】 Objective To analysis the propofol joint fentanyl intravenous anesthesia performed in abortion looking for suitable application effect in comprehensive nursing measures.Methods Select our hospital.all August 2009 to 2010 psychomotor agitation may execute abortion art pregnant women,according to the principle of 90 cases randomly divided into research group(groupA,n=45cases),and the control group(group B,n=45 patients),the team performed abortion pregnant women give vein push note fentanyl(1~2ug/kg)and propofol(2mg/kg);The control group simply using propofol intravenous(2mg/kg),observation compared between the two groups anaesthetic effect,and cervical relaxation degree,forgotten degree,operative and mean arterial pressure(MAP),heart rate(HR),breathing(R),blood oxygen saturation(SpO2,etc.).It also summarizes implement comprehensive nursing measures related experience.Results Two groups are satisfactory pregnant analgesic effect,the team compared with controls found that palace mouth flabby,forgotten degree and operation time was better than control group(P<0.05).Conclusion Propofol joint fentanyl intraoperative for abortion with satisfactory effect,play analgesic action at the same time,adverse reaction less,safe and effective,and clinical application.Meanwhile nursing measures implementation need to pay attention to strengthen respiration,circulation guardianship and postoperative narcotic observation.
