应用Embophere微球栓塞治疗急性大咯血的临床研究

【摘要】 目的研究Embophere微球在支气管动脉栓塞治疗急性大咯血中的价值。资料与方法回顾分析2009年5月~2009年12月,27例急性大咯血患者行Emophere微球栓塞治疗的临床资料。予选择性支气管动脉造影后,用微导管超选择插至支气管动脉,注入不同规格的Embophere微球栓塞末梢支气管动脉,并继以明胶海绵颗粒加强栓塞,观察治疗有效率及并发症情况。结果 26例获显著疗效(96.3%),其中即刻止血23例(85.2%)、1d~3d内咯血停止3例(11.1%)。1例(3.7%)即刻出血有缓解,但1周后复发,行手术治疗。随访(6~9)个月,除以上1例手术切除外,无复发。6例术后出现胸痛,均在一周内缓解,未见截瘫等严重并发症。结论 Embophere微球在支气管动脉栓塞治疗急性大咯血安全、有效,值得临床推广应用。

【Abstract】 Objective To evaluate the value of application of Embosphere microphere in bronchial artery embolization for severe hemoptysis.Materials and methods Twenty-seven patients of severe hemoptysis were treated with embolization consecutively between May and December,2009.After selective bronchial arteriography,micro-catheters were employed to cannulate the distal end of bronchial arteriy,then the Embosphere was injected followed by gelatine sponge particles.Its efficacy and complications were analyzed retrospectively.Result In 27 cases,26(96.3%) had significant effect,in which 23 of bleeding ceased instantly after embolization and 3 in 1-3 days after embolization.One received surgical operation because of recurring after 1 week,even though immediate alleviation of bleeding.In follow-up period of 6-9 months,no one of hemoptysis recurred except for the one mentioned above.Chest pain was observed in 6 patients after embolization,which was relieved in 1 week.No severe complications such as paraplegia happened.Conclusion Bronchial artery embolization with the application of Embosphere is effective and safe in the treatment of severe hemoptysis.
