联合应用18F-FDG-SPECT/CT和131I-SPECT/CT监测分化型甲状腺癌复发和转移灶的价值
Value of Combined Application of 18F-FDG-SPECT/CT and 131I-SPECT/CT in Detection of Recurrence or Distant Metastases in Patients with Differentiated Thyroid Cancinoma
【摘要】 目的:探讨联合应用18F-FDG-SPECT/CT和131I-SPECT/CT监测分化型甲状腺癌复发和转移灶的价值。方法:对89例DTC术后131I去除治疗后患者行131I-WBS、131I-SPECT/CT、18 F-FDG-SPECT/CT检查,以病理及随访半年以上结果为标准,将影像结果分类,评价131I-WBS、131I-SPECT/CT、18 F-FDG-SPECT/CT的诊断价值。结果:131I-WBS检查发现DTC转移灶34例,假阴性2例,假阳性病灶13例。131I-SPECT/CT融合显像检查发现DTC转移灶25例,假阴性3例,假阳性病灶4例。18 F-FDG-SPECT/CT检查发现DTC转移灶24处,没有假阴性及假阳性病例,但良性病变(残留甲状腺)出现4例假阴性。结论:联合应用18F-FDG-SPECT/CT显像和131I-SPECT/CT融合图像能准确检出、定位DTC转移灶和除外假阳性结果,有益于DTC患者复发和转移灶的诊断和鉴别诊断。
【Abstract】 Objective:To investigate the value of combined application of 18F-FDG-SPECT/CT and 131I-SPECT/CT fusion images in the detecting recurrence or metastases in patients with differentiated thyroid carcinoma(DTC).Methods:89 patients with DTC underwent 131I-WBS、131I-SPECT/CT、18F-FDG-SPECT/CT.Taking the histopathologic findings and clinical and imaging follow-up within 6 months for the gold standard,the diagnositic value of 131I-WBS、131I-SPECT/CT、18F-FDG-SPECT/CT were evaluated.Results:There were 34 cases of positive findings with 131I-WBS.2 cases of the metastatic lesions showed the negative in 131I-WBS.There were 13 false positive cases.There were 25 cases of positive findings with 131I-SPECT/CT fusion images examination.2 cases of the metastatic lesions showed the negative in 131I-SPECT/CT.There was 4 false positive cases.There were 24 cases of positive findings with 18F-FDG-SPECT/CT fusion images examination.There was no false positive case or negative result.But there were 4 cases of false positive finding of the residual thyroid tissue.Conclusion:Combined application of 18F-FDG-SPECT/CT and 131I-SPECT/CT fusion images can detect and locate the metastasis and exclude the false positive results accurately,which is a good method to detect the false and metastases in differentiated thyroid carcinoma(DTC).
