体重指数与食管癌切除术后预后之间的关系

【摘要】 目的探讨体重指数与食管癌切除术后预后之间的关系及其临床应用价值。方法 264例食管癌切除术患者,根据体重指数的不同,分为消瘦、体重正常、超重和肥胖四组,比较各组患者短期预后(并发症发生率、再手术率和住院死亡率)和远期预后(5年生存率)的差异。结果所有患者术后并发症总发生率为33.3%,再手术率为12.1%,住院死亡率为3.4%,各组差异均无统计学意义。术后并发吻合漏的发生率随体重指数的增大而增加。所有患者5年生存率为28.8%,采用生存曲线log-Rank检验各组患者5年生存率差异无统计学意义。结论体重指数对食管癌术后预后影响小,不能作为病人是否选择手术切除的参考依据。

【Abstract】 Aim To investigate whether the body mass index(BMI)is of prognostic value with regard to the short-term and long-term outcome in patients who undergo esophagectomy for cancer.Methods In 264 patients who underwent esophagectomy,the short-term(complication rate,reoperation rate,hospital mortality)and long-term(5-year survival rate)were compared with different BMI classes(underweight,normal weight,overweight,obesity).Results The total complication rate was 33.3%,the reoperation rate 12.1%,the hospital mortality positive rate 3.4%,and there were no statistical difference between different BMI classes.The risk for anastomotic leakage increased with higher BMI.The 5-year survival rate of total patients was 28.8%,and there was no statistical difference between different BMI classes.Conclusion BMI is not of prognostic value with regard to short-term and long-term outcome in patients who undergo esophagectomy for cancer and is not an independent predictor for radical resection.
