【摘要】 目的探索胰岛细胞抗体、谷氨酸脱羧酶抗体和酪氨酸磷酸酶样蛋白抗体联合检测,对成人糖尿病免疫学诊断、鉴别诊断的临床意义。方法随机选取新诊断的成人糖尿病患者786例,用ELISA法分别检测患者血清GAD-Ab、ICA、IA-2A,比较单独检测和联合检测阳性率的差异。结果 786例血清胰岛细胞自身抗体ICA、GADA和IA-2A阳性率分别为18.2%、10.5和17.9%;3种胰岛细胞自身抗体联合检测可提高LADA的诊断率;20~45岁新诊断成人糖尿病患者的胰岛细胞自身抗体ICA、GADA和IA-2A联合检测阳性率高于46~65岁组。结论新诊断成人糖尿病血清胰岛细胞自身抗体(ICA、GADA和IA-2A)联合检测有利于2型糖尿病中区分出自身抗体阳性的成人糖尿病,20~45岁组的胰岛细胞自身抗体的阳性率高于46~65岁组。更多还原

【Abstract】 Objective To explore the clinical value of the investigation of three islet autoantibody in newly diagnosed adult patients with DM.Methods To randomly select 786 patients newly diagnosed DM and detected GAD-Ab、ICA、IA-2A by ELISA,compared the positive rate of difference between single and combined detection.Results The positive rates of GAD-Ab、ICA、IA-2A of these patients were 18.2%,10.5% and 17.9%,respectively.The positive rate of combined detection with GAD-Ab、ICA and IA-2A was significantly higher than that of single detection.And the positive rate of combined detection in 20~45 years old group was significant higher that of 46~65 years old group patients.Conclusion The combination detection of islet autoantibody can increase the diagnose rate of adult diabetes in newly diagnosed patients with diabetes.The positive rate islet autoantibody in 20~45 years old group was higher than that in 46~65 years old group.更多还原
