通过疾病监测数据评估甲型H1N1流感流行的方法探讨
Evaluation Methods of Influenza A /H1N1 Pandemic by Disease Surveillance Data
【摘要】 目的探讨利用现有的疾病监测系统,及时准确评估在某一时段内甲型H1N1流感的流行强度和特点。方法通过流感哨点医院监测系统、流感网络实验室监测系统、疾病死亡监测系统监测数据与往年平均水平的比较,计算因甲型H1N1流感造成的超额流感样病例发病数和死亡数。结果根据流感哨点监测数据计算出从36~42周(2009年9月17日—10月27日)累积超额流感样病例51447人;根据流感网络实验室数据得出甲型H1N1阳性标本占全部阳性标本的比率持续上升,在43周后达到100%;根据疾病死亡监测数据计算出60岁以上人群因肺炎死亡较2006—2008年同期相比超额死亡55人。结论通过监测系统中计算得出的积累超额流感样病例数、甲型H1N1流感在流感阳性病例所占比例、因肺炎超额死亡数,综合评估5~25岁年龄组人群流感样病例成为甲型H1N1流感主要感染人群,高年龄组人群(60岁以上)病死率可达2.6%,儿童、青壮年病死率仅为万分之一。
【Abstract】 [Objective]To evaluate epidemic intensity and characteristics of Influenza A/H1N1 with the existing disease surveillance system.[Methods]Based on influenza sentinel hospital surveillance system,influenza network laboratories monitoring system,disease surveillance system for monitoring mortality,the monitoring data were used to calculate excess influenza cases and deaths caused by influenza A /H1N1 through comparison of the average in previous years.[Results]According to influenza sentinel surveillance data from 36th~42nd week (September17—October 27),there were 51 447 cases of cumulative excess of influenza-like illness;the network of laboratory data indicated that the ratio of influenza A/H1N1 continued to rise,and reached 100% at the end of 43rd week;there were 55 excess pneumonia deaths in population over 60 years comparing to the average between 2006—2008.[Conclusion]Through monitoring the system,the accumulation of excess number of influenza-like cases,proportion of Influenza A /H1N1 and the excess deaths due to pneumonia are calculated.The comprehensive evaluation indicated that 5~25 years age group is the major infection group for influenza A/H1N1,the fatality rate is up to 2.6% in patients over 60 years,but the fatality rate in children and young adults is only1/10 000.
