畲汉人群ACE基因多态性与原发性高血压关系的病例对照研究

【摘要】 目的探索浙江省畲汉两族人群ACE基因多态性分布情况及其与原发性高血压的关系。方法采用聚合酶链反应(PCR)检测215例畲汉两族原发性高血压(EH)及正常人群血中ACE基因16内含子I/D多态标记,分析计算各基因型(DD型、ID型、Ⅱ型)及等位基因频率,运用Logistic多元回归分析ACE基因型与原发性高血压的关系。结果浙江畲族EH组与对照组DD、ID、Ⅱ基因型频率分别为39.7%、32.8%、27.6%及18.8%、45.8%、35.4%,D和I等位基因分别为56.0%、44.0%及41.7%、58.3%。汉族EH组与对照组DD、ID、Ⅱ基因型频率分别40.7%、31.5%、27.8%及21.8%、40.0%、38.2%,D和I等位基因分别为56.5%、43.5%及41.8%、58.2%。Logistic回归显示DD基因型为原发性高血压危险因素(OR=2.746,P<0.05)。结论畲汉两族之间ACE基因多态性构成没有差异,两族中带有DD基因型及D等位基因的个体易患原发性高血压。

【Abstract】 Objective To describe the distribution of ACE gene polymorphism and explore its association with the risk of essential hypertension in She and Han nationalities in Zhejiang province. Methods The I/D polymorphism of intron 16 of ACE gene were detected among 112 cases with essential hypertension (EH) and 103 healthy controls of She and Han nationalities using polymerase chain reaction (PCR) technique. Frequencies of genotypes (DD,ID,Ⅱ) and alleles were then calculated. Meanwhile,multiple logistic regression was used to analyze the relationship between ACE genotypes and essential hypertension risk. Results In She nationality,the frequencies of DD,ID,and Ⅱ genotypes in EH and control group were 39.7%,32.8%,and 27.6% vs. 18.8%,45.8%,and 35.4%,respectively. And frequencies of D and I alleles were 56.0% and 44.0% vs. 41.7% and 58.3%,respectively. In Han nationality,the frequencies of DD,ID,and Ⅱ genotypes in EH and control group were 40.7%,31.5%,and 27.8% vs. 21.8%,40.0%,and 38.2% respectively. And frequencies of D and I alleles were 56.5% and 43.5% vs. 41.8% and 58.2%,respectively. In addition,Logistic regression indicated that DD genotype had an increasing risk of essential hypertension ( OR=2.746,P<0.05). Conclusion There is no difference in the distribution of ACE gene polymorphism between She and Han nationalities. ACE genetic polymorphism may be associated with the risk of essential hypertension in both She and Han nationalities.
